摘要
目的探讨益气活血通络汤联合阿司匹林预防儿童股骨转子间骨折下肢静脉血栓的临床效果。方法选取2015年1月—2018年12月医院收治的100例股骨转子间骨折儿童患者临床资料进行前瞻性研究,随机分为两组,各50例。对照组给予阿司匹林治疗,治疗组在阿司匹林治疗基础上给予益气活血通络汤治疗。比较两组患者临床指标、凝血指标、超声检查指标、炎症指标和不良反应发生情况以及患儿家属对本次治疗满意程度。结果治疗组患者红细胞压积变化率、血红蛋白变化率、72 h内引流量变化、输血次数、隐性失血量、人均输血量、深静脉血栓形成率、小腿肌间静脉丛血栓形成率、多血管血栓形成率与血栓总长度比对照组均降低,差异均有统计学意义(P<0.05)。治疗前,两组患者FIB、PLT、PT、APTT、D-D、CRP、TNF-α和IL-6相比,无明显差别(P>0.05);治疗后,两组患者FIB、PLT、D-D、CRP、TNF-α和IL-6水平比治疗前均降低,且治疗组FIB、PLT、D-D、CRP、TNF-α和IL-6水平降低幅度比对照组大,差异均有统计学意义(P<0.05)。治疗后,两组患者PT、APTT比治疗前均延长,且治疗组PT、APTT水平延长幅度比对照组大,差异均有统计学意义(P<0.05)。治疗组患儿家属本次治疗满意率显著高于对照组,差异具有统计学意义(P<0.05)。结论益气活血通络汤联合阿司匹林预防儿童股骨转子间骨折下肢静脉血栓的疗效确切,且可降低患者炎症反应,同时不增加不良反应发生率,提升家属满意率,降低医疗纠纷发生率,安全可靠,值得临床广泛推广以及研究。
Objective To investigate the clinical efficacy of Yiqi Huoxue Tongluo Decoction combined with aspirin in the treatment of venous thrombosis in children with intertrochanteric fractures.Method A total of 100 pediatric patients with intertrochanteric fracture admitted to our hospital from January 2015 to December 2018 were selected for prospective study,which were randomly divided into two groups with 50 cases in each.The control group was treated with aspirin,while the treatment group was treated with Yiqi Huoxue Tongluo Decoction on the basis of aspirin.The clinical indicators,coagulation indicators,ultrasound examination indicators,inflammatory indicators and adverse reactions of the two groups were compared.The satisfaction rate of the children’s families was also compared.Result Red blood cells deposited rate within 72 h,hemoglobin rate,flow changes,blood transfusion,the hidden blood loss,blood transfusion amount,per capita rate of deep vein thrombosis,calf muscular venous plexus thrombosis rate,rate of vascular thrombosis and thrombosis length in the treatment group were lower than those of the control group(P<0.05).Before treatment,there was no significant difference in FIB,PLT,PT,APTT,D-D,CRP,TNF-alpha or IL-6 between the two groups(P>0.05).After treatment,the levels of FIB,PLT,D-D,CRP,TNF-alpha or IL-6 in the two groups were all decreased compared with those before treatment,and the reduction in the treatment group was statistically greater than that in the control group(P<0.05).After treatment,levels of PT and APTT in both groups were prolonged compared with those before treatment,and those in the treatment group was statistically larger than that in the control group(P<0.05).The satisfaction rate of the family members of the children in the treatment group was significantly higher than that of the control group(P<0.05).Conclusion The efficacy of Yiqi Huoxue Tongluo Decoction combined with aspirin in preventing venous thrombosis of lower extremity in children with intertrochanteric fracture is definite,
