摘要
目的:应用7.0 T MRI分析14~22周胎儿标本脑外侧沟的基本形态变化规律。方法:收集46例14~22周胎儿标本行7.0 T MRI扫描,于T2WI轴位图像上对双侧外侧沟进行识别,并勾画其整体边缘轮廓,分析外侧沟在此时期随孕龄增长的基本形态变化。结果:轴位观察时,妊娠中期胎儿外侧沟形态于基底节区、丘脑层面最典型。外侧沟整体发育过程表现为由浅到深、表面开口由大到小的变化规律,即由皮质表面宽大而表浅的凹陷发育变化为狭窄并深入的缝隙。其中14~15周外侧沟呈浅波浪状,16~17周呈浅梯形状,18~19周呈深梯形状,20~22周呈π状。14~22周胎儿双侧外侧沟均未发生迂曲、折叠或发出次级分枝;右侧半球外侧沟凹陷程度与岛盖闭合速度较左侧略快。结论:胎儿外侧沟的整体形态发育变化与邻近岛盖皮质的发育、聚拢程度紧密相关;妊娠中期胎儿外侧沟尚处于发育的初始阶段;7.0 T MRI可清晰显示此时期胎儿标本外侧沟的形态变化。
Objective:To analyze morphological changes of the fetal sylvian fissure during 14 ~22 gestational weeks on the transverse 7.0 T post-mortem MRI.Methods:46 fetal specimens of 14~22 gestational weeks were scanned by 7.0 T MRI.The bilateral sylvian fissures were identified and their morphological outlines were manually delineated on the transverse T2 WI.The developmental changes of the sylvian fissure with gestational age during this period were obtained.Results:Morphology of the fetal sylvian fissure during the second trimester was the most typically described on the transverse MRI through the basal ganglia and thalamus.Developmental process of the sylvian fissure was described with its depth from shallow to deep and the surface opening from large to small,which changed from a wide and shallow fossa initially to a narrow and deep fissure finally on the cortical surface.The sylvian fissure was described as ’shallow wavy’ in 14~15 gestational weeks,’shallow trapezoid’ in16~17 gestational weeks,’deep trapezoid’ in 18~19 gestational weeks,and ’π’ shape in 20~22 gestational weeks,respectively.The bilateral fetal sylvian fissures were not tortuous,folded or developed the secondary branches during this period.The sylvian fissure appeared deeper with the opercula insulae approaching each other faster at the right hemisphere.Conclusions:Morphological development of the fetal sylvian fissure is closely related to the opercula insulae cortex and its approaching.The fetal sylvian fissure is at the initial developmental stage during the second trimester,and 7.0 T post-mortem MRI could clearly delineate its morphological changes.
