摘要
目的:分析创伤性骨化性肌炎的SPECT-CT特征,探讨SPECT-CT在创伤性骨化性肌炎中的应用价值。方法 :回顾性分析28例创伤性骨化性肌炎的SPECT-CT资料,均经手术病理或6个月以上的临床随访证实。结果:28例中,根据CT表现分为中期13例,其中7例病灶出现斑片状、絮样稍高密度影,4例病灶骨化有外周向中心呈高-稍高-等密度的分层现象,2例病灶边缘出现蛋壳样圆弧形钙化;晚期15例,表现为条状、团块状或不规则形的边界清楚的高密度骨块影,其中9例病灶外层为厚的环形致密钙化层,中央镂空。SPECT示19例呈团状放射性显像剂异常浓聚,但无法进行精确解剖定位。SPECT-CT融合图像示骨化未成熟16例,其中11例显像剂异常浓聚范围局部超过骨化范围,5例全部超过骨化范围;骨化成熟12例,其中9例骨化病灶内无明显显像剂异常浓聚,3例异常浓聚范围小于骨化病灶。结论:创伤性骨化性肌炎具有一定的SPECT-CT图像特征,能为其骨化程度的评估提供可靠依据,值得临床推广应用。
Objective:To analyze SPECT-CT features of traumatic myositis ossificans and explore the application value of SPECT-CT.Methods:SPECT-CT imaging data of 28 patients with traumatic myositis ossificans were analyzed retrospectively,and the application value of SPECT-CT in traumatic myositis ossificans was discussed based on surgical pathology or clinical follow-up over 6 months.Results:CT showed that there were 13 cases in the middle stage,7 cases showed patchy and flocculated lesions with slightly high density,4 cases showed high-slightly high-equal density stratification in the periphery toward the center of the lesions,and 2 cases showed "eggshell" like circular arc calcification at the edge of the lesions.In the late stage,15 cases showed striated,lumpy or irregular bone mass with high density and clear boundary,among which 9 exceptional layers were thick ring dense calcified layer with hollow center.SPECT showed a cluster of abnormal concentration of radionuclides in 19 cases,but not anatomic localization.The SPECT-CT fusion image showed that the ossification was immature in 16 cases,the concentration range of nuclide locally exceeded the ossification range in 11 cases,and 5 cases exceeded the ossification range.There were 12 cases with mature ossification,9 cases of ossification lesions were without obvious nuclide concentration,and 3 cases of ossification lesions had a smaller range of nuclide concentration.Conclusions:Traumatic myositis ossificans has certain SPECT-CT image characteristics,which can provide a reliable basis for evaluation of ossification degree and is worthy of clinical application.
