PET/CT-MRI三模式对头颈部肿瘤的诊断价值

摘要 目的:探讨PET/CT-MRI三模式技术在头颈部肿瘤诊断中的应用价值。方法:对47例临床疑诊头颈部肿瘤的患者行PET/CT-MRI三模式检查,其中45例加行多b值DWI检查。计算病灶标准摄取值(SUV)、慢速ADC(ADCslow)、快速ADC(ADCfast)及水通道蛋白ADC值(ADCAQP)。以病理为金标准,比较PET/CT、MRI及PET/CT-MRI三模式对头颈部病变的诊断价值,确定头颈部良恶性病变的最佳诊断界值。结果:PET/CT-MRI三模式在头颈部良恶性病变的诊断敏感度、阳性预测值、阴性预测值及准确率上较PET/CT、MRI有一定提高。SUVmax=6.65 g/mL与ADCAQP(mean)=0.212 5μm2/s为头颈部良恶性病变的最佳诊断界值。结论:PET/CT-MRI三模式对提高头颈部肿瘤的检出及诊断有一定意义。 Objective:To investigate the diagnostic value of trimodality PET/CT-MRI in head and neck tumors.Methods:47 cases of clinical suspect with head and neck tumors patients underwent trimodality PET/CT-MRI,among them 45 cases underwent mult-b value scan of MRI.The standardized uptake value(SUV),fast ADC(ADCfast),slow ADC(ADCslow) and aquaporins ADC(ADCAQP) of all lesions were caculated.The results were confirmed by pathology and then the diagnostic value of PET/CT,MRI and trimodality PET/CT-MRI were compared.The best diagnostic boundary of benign and malignant head and neck lesions were found.Results:Compared with PET/CT and MRI,trimodality PET/CT-MRI improved the sensitivity,post predictive value,negative predicitive value and accuracy in the diagnosis of benign and malignant lesions of head and neck.The best diagnostic boundary of SUVmaxwas 6.65 g/mL,the best diagnostic boundary of ADCAQP(mean) was 0.212 5 μm2/s.Conclusion:Compared with PET/CT and MRI,trimodality PET/CT-MRI has certain significance for the detection and diagnosis of head and neck diseases.
