摘要
目的探讨磁共振弥散张量成像(DTI)在评估青年急性缺血性脑梗死预后的作用。方法选取2017年8月至2019年2月于广州开发区医院就诊的青年急性缺血性脑梗死住院患者81例,经治疗后分为症状无改善组(无改善组) 20例及症状改善组(改善组) 61例,行常规头部MRI平扫、DWI、DTI扫描。对比治疗前后两组患者美国国立卫生研究院卒中量表(NIHSS)评分及简式Fugl-Meyer运动功能评分法(Fugl-Meyer评分);各向异性系数(FA)和表观扩散系数(ADC)值,及其与NIHSS评分及Fugl-Meyer评分的相关性。结果治疗后改善组患者NIHSS评分显著降低,而Fugl-Meyer评分显著升高(P<0.01);且治疗后改善组患者ADC值、FA值显著升高(P<0.01)。Pearson相关性分析发现ADC值、FA值随着NIHSS评分降低而升高,呈明显负相关(P<0.05),而ADC值与Fugl-Meyer评分无显著相关性(P>0.05),FA值与Fugl-Meyer评分呈明显正相关(P<0.01)。结论 DTI在评估青年急性缺血性脑梗死预后作用显著,FA值与NIHSS评分、Fugl-Meyer评分均具有明显相关性。
【Objective】To investigate the role of diffusion tensor imaging(DTI) in evaluating the prognosis of young patients with acute ischemic cerebral infarction.【Methods】From August 2017 to February 2019, 81 young patients with acute ischemic cerebral infarction within 72 hours of onset were recruited for routine head MRI plain scan, DWI and DTI scan. The improvement of clinical symptoms, anisotropic coefficient(FA) and apparent diffusion coefficient(ADC) were compared before and after treatment,and their correlation was analyzed.【Results】NIHSS score was significantly decreased, while Fugl-Meyer score was significantly increased(P<0.01), and the ADC and FA values were significantly increased after treatment(P<0.01). Pearson correlation analysis showed that ADC value and FA value increased with the decrease of NIHSS score, and had a significant negative correlation(P<0.05),while ADC value had no significant correlation with Fugl-Meyer score(P>0.05), and FA value had a significant positive correlation with Fugl-Meyer score(P<0.01).【Conclusion】DTI plays an important role in evaluating the prognosis of young patients with acute ischemic cerebral infarction. FA value is significantly correlated with Fugl-Meyer score and NIHSS score.
