扩散加权成像及扩散张量成像在肾透明细胞癌核分级中的价值 被引量:2

Diffusion Weighted Imaging and Diffusion Tensor Imaging in Evaluating the Nuclear Grade of Clear Cell Renal Cell Carcinoma

摘要 目的探索磁共振扩散加权成像(DWI)及扩散张量成像(DTI)在评价肾透明细胞癌(ccRCC)核分级中的价值。资料与方法回顾性分析经手术病理证实的42例ccRCC患者,均行腹部MRI检查。根据病理结果Fuhrman分级分为3组:A组(Ⅰ级、Ⅰ~Ⅱ级)14例,B组(Ⅱ级)16例,C组(Ⅲ、Ⅲ~Ⅳ级)12例。3组患者均行1.5T MRI常规T1WI、T2WI扫描、LAVA增强扫描、DWI、DTI序列扫描。采用ADW 4.6工作站的Functool后处理软件生成DWI的表观扩散系数(ADC)图及DTI的ADC图和各向异性分数(FA)图,在相应ADC图和FA图中的ccRCC实质部分放置感兴趣区(ROI),测量各ROI的ADCDWI值、ADCDTI值及FA值。比较在ccRCC 3个不同分化组之间ADCDWI值、ADCDTI值及FA值的差异。结果3组间ADCDWI值和ADCDTI值差异均有统计学意义(P≤0.01),且C组ADCDWI值和ADCDTI值均低于A组和B组(P<0.05)。3组间FA值差异无统计学意义(P>0.05)。结论DWI及DTI的ADC值能有效评价ccRCC的核分级情况。 Purpose To explore the value of magnetic resonance diffusion weighted imaging(DWI)and diffusion tensor imaging(DTI)in the evaluation of nuclear grade of renal clear cell carcinoma(ccRCC).Materials and Methods A retrospective analysis of 42 ccRCC patients confirmed by surgical pathology was performed with abdominal MRI.According to the pathological results,Fuhrman classification was divided into three groups:14 cases in group A(grade Ⅰ,grade Ⅰ-Ⅱ),16 cases in group B(gradeⅡ),12 cases in group C(gradeⅢ,Ⅲ-Ⅳ).All patients in the three groups underwent 1.5T MRI routine T1WI,T2WI scan,LAVA enhanced scan,DWI and DTI sequence scan.Functool post-processing software of the ADW 4.6 workstation was used to generate the DWI apparent diffusion coefficient(ADC)graph and DTI ADC graph and anisotropy score(FA)graph.For every ROI,the ADC values and FA values were measured and analyzed.ROI was set in the substantial part of the ccRCC in the corresponding ADC graph and FA graph,the ADCDWI,ADCDTI and FA of each ROI were measured.The difference of ADCDWI,ADCDTI and FA among three different differentiation groups of ccRCC were compared.Results The differences in ADCDWI and ADCDTI values among the three groups were statistically significant(P≤0.01),and the ADCDWI and ADCDTI values in group C were lower than those in groups A and B(P<0.05).There was no statistically significant difference in FA values between the three groups(P>0.05).Conclusion The ADC values of DWI and DTI can effectively evaluate the nuclear classification of ccRCC.
