摘要
目的探讨股骨近端防旋髓内钉、股骨近端锁定加压钢板治疗老年股骨转子间骨折的效果。方法选取本院2017年6月至2018年6月收治的60例老年股骨转子间骨折患者作为研究对象,按手术方法不同分为股骨近端防旋髓内钉(PFNA)组(n=32)和股骨近端锁定加压钢板(PFLCP)组(n=28)。比较两组手术情况、术后恢复情况、并发症发生率。结果 PFNA组术中出血量少于PFLCP组(P<0.05);PFNA组手术时间、住院天数、骨折愈合时间均短于PFLCP组(P<0.05);术后6个月,PFNA组髋关节功能(Harris)评分高于PFLCP组(P<0.05);术后1年,两组Harris评分比较差异无统计学意义;PFNA组术后并发症发生率(6.25%)低于PFLCP组(28.57%)(P<0.05)。结论在老年股骨转子间骨折治疗中,PFNA可显著减少术中出血量,缩短手术时间,降低并发症发生率,加快患者恢复速度,提高骨折愈合率,尤其适用于骨质疏松老年患者,可作为老年股骨转子间骨折的首选手术方式。
Objective To explore the curative effect of proximal femoral nail anti-rotation(PFNA) and proximal femoral locking compression plate(PFLCP) on intertrochanteric fractures in the elderly. Methods 60 elderly patients with intertrochanteric fractures in our hospital from June2017 to June 2018 were enrolled as the study subjects. According to different surgical methods, they were divided into PFNA group(n=32) and PFLCP group(n=28). The operation status, postoperative recovery and incidence of complications were compared between the two groups. Results The intraoperative blood loss in PFNA group was less than that in PFLCP group(P<0.05). The operation time, hospitalization time and fracture healing time in PFNA group were shorter than those in PFLCP group(P<0.05). At 6 months after surgery, score of hip function(Harris) in PFNA group was higher than that in PFLCP group(P<0.05). At 1 year after surgery, there was no significant difference in Harris score between the two groups. The incidence of postoperative complications in PFNA group was lower than that in PFLCP group(6.25% vs. 28.57%)(P<0.05). Conclusion In the treatment of intertrochanteric fractures in the elderly, PFNA can significantly reduce intraoperative blood loss, shorten operation time, reduce incidence of complications, speed up patients recovery, and improve fracture healing rate, which is suitable for elderly patients with osteoporosis. At present, the operation method is preferred for elderly intertrochanteric fractures.
