摘要
目的探讨彩色多普勒超声在巨大卵巢囊腺瘤疾病诊断中的临床价值及意义。方法选取2012年1月至2017年12月本服务中心门诊和参加国家免费婚孕健康体检的适龄服务对象(均有性生活史)进行超声检查,16例巨大卵巢囊腺瘤患者,以病理结果作为金标准,观察子宫及附件,注意肿块大小、范围、内部结构及血流信号特征等。结果 16例患者均经手术及病理证实。浆液性囊腺瘤9例,占56.2%(9/16),其中4例呈单纯性,占44.4%(4/9);5例呈乳头状,占55.5%(5/9)。黏液性囊腺瘤7例,占43.7%(7/16),呈多房性;诊断符合率87.5%(14/16),误诊2例,误诊率12.5%(2/16)。CDFI:瘤体内囊性部分呈无血流信号,低回声实质内、囊壁及分隔光带上见少许血流信号,呈低速、中等阻力动脉频谱。结论彩色多普勒超声诊断巨大卵巢囊腺瘤操作简单、方便、无创伤,可鉴别良、恶性肿瘤,诊断符合率较高,可作为术前检查及疗效评估,是临床上重要的一种辅助检查手段。
Objective To investigate the clinical value and significance of color Doppler ultrasound in the diagnosis of giant ovarian cystadenoma. Methods From January 2012 to December 2017, the out-patient of the service center and the age-appropriate service objects(with sexual life history) participating in the national free marriage and pregnancy health examination were selected for ultrasound examination. The pathological results were used as the gold standard. The uterus and attachments were observed. The size, extent, internal structure and blood flow signal characteristics of the tumor were noted. Results All 16 patients were confirmed by surgery and pathology. 9 cases of serous cystadenoma, accounting for 56.2%(9/16),of which 4 cases were simple, accounting for 44.4%(4/9). 5 cases were papillary, accounting for 55.5%(5/9). Mucus 7 cases of cystadenoma, accounting for 43.7%(7/16), were multi-atrial. The diagnostic coincidence rate was 87.5%(14/16), 2 cases were misdiagnosed, and the misdiagnosis rate was12.5%(2/16). CDFI: There is no blood flow signal in the cystic part of the tumor. A few blood flow signals are seen in the hypoechoic parenchyma, the cyst wall and the separated light band, showing a low-speed, moderate-resistance arterial spectrum. Conclusion Color Doppler ultrasonography is a simple and convenient method for the diagnosis of giant ovarian cystadenoma. It can identify benign and malignant tumors and has a high diagnostic coincidence rate. It can be used as a clinically important one for preoperative examination and evaluation of curative effect. Auxiliary inspection means.
