摘要
目的研究焦点式心理干预(FPI)对老年慢性阻塞性肺疾病(COPD)患者运动能力、肺功能及自我管理能力的影响。方法纳入2015年7月至2019年6月我院收治的老年COPD患者146例,依据随机数字表法分为FPI组(n=73)与对照组(n=73),对照组给予常规干预,FPI组在对照组基础上给予FPI措施。观察两组干预前后焦虑、抑郁评分,运动能力,第1秒用力呼气容积(FEV1)、用力肺活量(FVC)及FEV1/FVC等肺功能指标,症状、信息、情绪、日常生活及自我效能等自我管理能力指标。结果干预后,FPI组SAS、SDS评分均低于对照组,6分钟步行实验(6MWT)评分高于对照组(P<0.05)。FPI组FEV1、FVC及FEV1/FVC高于对照组(P<0.05)。FPI组症状、信息、情绪、日常生活及自我效能管理评分均高于对照组(P<0.05)。结论FPI可有效缓解老年COPD患者心理痛苦,解除其焦虑、抑郁情绪,提高其运动能力、肺功能及自我管理能力。
Objective To investigate the effects of focal psychological intervention(FPI)on exercise ability,lung functions and self-management ability in elderly COPD patients.Methods We selected 146 elderly patients with COPD admitted to our hospital from July 2015 to June 2019.The patients were divided into FPI or control group using random number table method,73 in each group.Conventional intervention was given to the control group,and on this basis,FPI was additionally given to the FPI group.Anxiety,depression scores,athletic ability,lung function indicators including 1 second forced expiratory volume(FEV1),forced vital capacity(FVC),and FEV1/FVC,and self-management ability indicators including symptoms,information,mood,daily life and self-efficacy were observed in the two groups.Results After intervention,SAS scores and SDS scores in the FPI group were lower than those in the control group,and the 6 MWT ability scores was higher than that in the control group(P<0.05).FEV1,FVC and FEV1/FVC in the FPI group were higher than those in the control group(P<0.05).Symptoms,information,mood,daily life and self-efficacy in the FPI group were higher than those in the control group(P<0.05).Conclusion FPI can effectively alleviate the psychological pain of elderly COPD patients,relieve their anxiety and depression,and improve their exercise ability,lung functions and self-management ability.
