摘要
目的探讨在食管癌手术中应用管状胃食管吻合治疗的临床效果。方法选取我院2014年2月~2019年2月收治的75例食管癌患者为研究对象,采用随机数字表法分为参照组与实验组,在食管癌切除术基础上,参照组应用全胃代食管吻合术治疗,实验组应用管状胃食管吻合术治疗;比较两组手术情况与住院时间、治疗前后肺功能、生活质量及并发症情况。结果两组术中出血量、手术时间、住院时间比较,差异无统计学意义(P>0.05);治疗前,两组患者肺活量占预计值(FVC%)、一秒钟用力呼气容积占预计值(FEV1%)、最大通气量占预计值(MVV%)比较,差异无统计学意义(P>0.05),治疗后,实验组患者FVC%、FEV1%、MVV%指标明显高于参照组,差异有统计学意义(P<0.05);两组患者术前SF-36评分比较,差异无统计学意义(P>0.05),术后2个月,实验组SF-36评分相较于参照组明显较高,差异有统计学意义(P<0.05);实验组患者并发症发生率(10.53%)略低于参照组(16.22%),但差异无统计学意义(P>0.05)。结论管状胃食管吻合术在食管癌手术中的实践效果较为理想,值得应用。
Objective To explore the clinical effect of tube gastroesophageal anastomosis in the operation of esophageal cancer.Methods 75 patients with esophageal cancer treated in our hospital from February 2014 to February 2019 were randomly divided into reference group and experimental group.On the basis of resection of esophageal cancer,the reference group was treated with total gastroesophageal anastomosis,and the experimental group was treated with tubular gastroesophageal anastomosis,and the surgical status,hospitalization time,lung function,quality of life and complications before and after treatment were compared between the two groups.Results There was no significant difference in intraoperative bleeding,operation time and hospitalization time between the two groups(P>0.05);Before treatment,the two groups had no significant difference in lung capacity(FVC%),one second force expiratory volume(FEV1%),maximum ventilation(MVV%),and no significant difference in lung capacity(P>0.05).After treatment,FVC%,FEV1%and MVV%in the experimental group were significantly higher than those in the reference group,the difference was statistically significant(P<0.05);The SF-36 scores of the two groups before operation were compared,There was no significant difference(P>0.05),two months after the operation,the SF-36 score in the experimental group was significantly higher than that in the reference group,the difference was statistically significant(P<0.05);The rate of complications in the experimental group(10.53%)was slightly lower than that in the reference group(16.22%),but the difference was not statistically significant(P>0.05).Conclusion The effect of the tube gastroesophageal anastomosis in the operation of esophageal cancer is ideal,and it is worth applying.
