摘要
目的探讨超声介入活检诊断乳腺叶状肿瘤(PTB)的价值。方法抽取2016年4月至2018年6月大同煤矿总医院收治的经手术病理学检查确诊的PTB女性患者48例,均行超声介入活检。48例患者年龄30~65岁,PTB良性36例、交界性7例、恶性5例。分析患者超声影像表现,并统计超声介入活检与手术病理诊断结果的符合情况。结果超声介入活检诊断PTB的准确度为95.83%(46/48),PTB良性、交界性、恶性诊断符合比例分别为34/36(94.44%)、7/7、5/5。34例PTB良性及9例PTB交界性者超声影像表现为肿瘤呈分叶状、长径<5.0 cm、内部回声为实性、边缘清晰、无或少量血流信号,5例PTB恶性者肿瘤则呈圆形或椭圆形、长径≥5.0cm、内部回声为囊实性、边缘模糊、血流信号丰富。结论超声介入活检诊断PTB的准确度较高,能提供肿瘤位置、长径、形态等影像学信息,可为后续的手术治疗提供依据。
Objective To investigate the diagnostic value of interventional ultrasound biopsy for phyllode tumor of breast (PTB).Methods Forty-eight female patients with PTB diagnosed by surgical pathological examination from April 2016 to June 2018 in Datong Coal Mine General Hospital were selected. Forty-eight patients were 30 to 65 years old, 36 cases were benign PTB, 7 cases were borderline, and 5 cases were malignant. The ultrasound imaging manifestations of patients were analyzed, and the accuracy between the results of ultrasound interventional biopsy and surgical pathological diagnosis were calculated.Results The accuracy of ultrasound interventional biopsy in the diagnosis of PTB was 95.83% (46/48), and the proportions of benign, borderline, and malignant diagnostis of PTB were 34/36 (94.44%), 7/7, and 5/5. The ultrasound images of 34 patients with benign PTB and 9 patients with bordering PTB showed lobulated tumors, major diameter <5.0 cm, solid internal echo, clear edges, no or small blood flow signals, and 5 patients with PTB malignant tumors were round or oval, with a length of ≥5.0 cm, the internal echos were cystic, the edges were blurred, and the blood flow signals were abundant.Conclusions Ultrasound interventional biopsy has high accuracy in the diagnosis of PTB, and can provide imaging information such as location, long diameter, and morphology of tumor, and can provide basis for subsequent surgical treatment.
