摘要
目的探讨经侧裂显微手术治疗高血压脑出血的有效性及安全性。方法抽取2016年1月至2019年9月于平煤神马集团总医院诊治的高血压脑出血患者106例作为研究对象,应用随机数字表法将其分为对照组与观察组,每组53例。对照组应用小骨窗开颅手术进行治疗,观察组应用经侧裂显微手术。比较两组患者的临床疗效。结果观察组病情恢复率[98.11%(52/53)]高于对照组[62.26%(33/53)],P<0.05;观察组重度致残率[1.89%(1/53)]低于对照组[33.96%(18/53)],P均<0.05。观察组健康指数、情感得分、社会指数、生活满意度等生活质量指标评分均优于对照组(P均<0.05)。观察组应激性溃疡、脑积水、再出血等不良反应发生率[3.77%(2/53)]低于对照组[15.09%(8/53)],P<0.05。结论应用经侧裂显微手术治疗高血压脑出血,能够有效缓解患者的临床症状,对患者生活质量的提升具有积极意义,且术后不良反应发生率低。
Objective To investigate the efficacy and safety of transsylvian approach microsurgery in the treatment of patients with hypertensive intracerebral hemorrhage.Methods A total of 106 patients with hypertensive intracerebral hemorrhage who were treated in Pingmei Shenma Group General Hospital from January 2016 to September 2019 were enrolled and divided into the control group and observation group by random number table method, with 53 cases in each group. Patients in the control group were treated by small bone window craniotomy, and patients in the observation group were treated by transsylvian approach microsurgery. The treatment outcomes of the two groups were recorded and compared.Results After treatment, the recovery rate of the observation group was 98.11% (52/53), higher than the 62.26% (33/53) of the control group (P<0.05). However, the severe disability rate was 1.89% (1/53) in the observation group, lower than the 33.96% (18/53) in the control group (P<0.05). Compared with the control group, the observation group had better outcomes in the health index, emotional score, social index, life satisfaction and other quality of life indicators (P<0.05). The incidence of adverse reactions, such as stress ulcer, hydrocephalus and re-bleeding, in the observation group was 3.77%(2/53), lower than the 15.09% (8/53) in the control group (P<0.05).Conclusions In the clinical treatment of patients with hypertensive intracerebral hemorrhage, transsylvian approach microsurgery can alleviate symptoms, promote quality of life, with low incidence of adverse reactions.
