摘要
目的:研究经尿道前列腺钬激光剜除术与前列腺等离子电切术对良性前列腺增生患者术后尿失禁发生率的影响。方法:2017年6月-2019年6月收治良性前列腺增生患者80例,随机分为两组,各40例。参照组采用前列腺等离子电切术。试验组采用经尿道前列腺钬激光剜除术治疗。比较两组治疗效果。结果:试验组手术时间及留置导管时间均短于参照组,术中出血量少于参照组,试验组拔除导尿管后24 h、1周和1个月尿失禁发生率均低于参照组,试验组拔除导尿管后24 h尿失禁发生程度优于参照组,差异均有统计学意义(P<0.05)。两组拔除导尿管后1周和1个月术后尿失禁程度比较,差异无统计学意义(P>0.05)。结论:在对良性前列腺增生患者进行治疗时,可采用经尿道前列腺钬激光剜除术与前列腺等离子电切术,其中经尿道前列腺钬激光剜除术后尿失禁发生率较低,拔除导尿管后24 h尿失禁发生程度比较好,有利于术后快速恢复,改善生活质量。
Objective:To study the effects of transurethral holmium laser enucleation of prostate and plasma prostatectomy on the incidence of postoperative urinary incontinence in patients with benign prostatic hyperplasia.Methods:From June 2017 to June 2019,80 patients with benign prostatic hyperplasia were selected,they were randomly divided into the two groups with 40 cases in each group.The control group was treated with plasma resection of prostate.The experimental group was treated with transurethral holmium laser enucleation of the prostate.The therapeutic effects of the two groups were compared.Results:The operation time and indwelling catheter time in the experimental group were shorter than those in the control group,and the amount of intraoperative bleeding in the experimental group was less than that in the control group.The incidence of urinary incontinence at 24 hours,1 week and 1 month after catheter removal in the experimental group was lower than that in the control group.The degree of urinary incontinence in the experimental group was better than that in the reference group 24 hours after the catheter was removed,the differences were statistically significant(P<0.05).There was no significant difference in the degree of urinary incontinence between the two groups 1 week and 1 month after catheter removal(P>0.05).Conclusion:Transurethral holmium laser enucleation of prostate and plasma resection of prostate can be used in the treatment of patients with benign prostatic hyperplasia,in which the incidence of urinary incontinence after transurethral holmium laser enucleation of prostate is low,the incidence of 24-hour urinary incontinence after the removal of catheter is better,which is beneficial to the rapid recovery after operation and improve the quality of life.
