摘要
目的对比观察超广角激光扫描检眼镜(scanning laser ophthalmoscope,SLO)和传统彩色眼底照相(color fundus photography,CFP)在年龄相关性白内障患者术前对视网膜出血的检出情况。方法采用诊断性试验研究方法,收集2018年3月至2019年2月在深圳市眼科医院行白内障超声乳化摘出联合人工晶状体(IOL)植入术的年龄相关性白内障患者204例360眼的临床资料。所有患眼均在白内障术前行视觉质量分析系统(OQASTMⅡ)、传统CFP以及超广角SLO检查,术后行散瞳后前置镜检查。根据OQASTMⅡ获得的客观散射指数(object scatter index,OSI)对不同混浊程度的晶状体进行分级。透过3种不同混浊程度的晶状体,对比观察传统CFP和超广角SLO眼底成像的图片质量评分以及两种检查设备对视网膜出血检出与白内障术后确诊的阳性符合率。结果在3种不同晶状体混浊程度下,超广角SLO的图片质量评分均高于传统CFP,差异均具有统计学意义(P=0.003、0.024、0.000)。OSI 1.0~<4.0为早期白内障,共100眼,术后实际确诊视网膜出血23眼。术前超广角SLO检出视网膜出血19眼,符合率为82.6%;传统CFP检出11眼,符合率为47.8%。OSI 4.0~<7.0为进展期白内障,共200眼,术后实际确诊视网膜出血35眼。术前超广角SLO检出视网膜出血20眼,符合率为57.1%;传统CFP检出12眼,符合率为34.3%。OSI≥7.0为成熟期白内障,共60眼,术后实际确诊视网膜出血11眼。术前超广角SLO检出视网膜出血4眼,符合率为36.4%;传统CFP检出2眼,符合率为18.2%。透过不同混浊程度的晶状体,超广角SLO视网膜出血检出符合率均高于传统CFP,差异均有统计学意义(P=0.000、0.000、0.026)。结论在年龄相关性白内障患者的术前眼底检查中,超广角SLO对于视网膜出血的检出符合率高于传统CFP。
Objective To determine the positive results of ultra-wide field scanning laser ophthalmoscope(SLO)and traditional color fundus photography(CFP)in screening retinal hemorrhage before surgery for cataracts.Methods A diagnostic test research study was performed.The clinical data of 360 eyes from 204 age-related cataract patients who received phacoemulsmcation with intraocular lens(IOL)implantation were collected in the Shenzhen Eye Hospital from March 2018 to February 2019.All subjects were underwent optical quality analysis systemTMⅡ(OQASTMⅡ)and fundus imaging using SLO and CFP before operation.Postoperatively,retina of these patients was examined by preplaced-mirror ophthalmoscope.An objective classification of cataracts were determined by the values of object scatter index(OSI)obtained by OQASTMⅡ.Through the different turbidity of lens,we analyzed the imaging quality of two technologies and compared according to the evaluation standards.The positive accordance rate of detection for retinal hemorrhage was used to compare between these two fundus imaging and post cataractous operation.Results The imaging quality scores of SLO were higher than those of CFP in the different turbidity of lens,and the difference was statistically significant(P=0.003,0.024,0.000).Early cataract is considered when the OSI value is between 1.0 and 4.0,which contains 100 eyes(retinal hemorrhage was actually diagnosed in 23 eyes after surgery).The preoperatively positive number of SLO was 19 eyes,with posi-tive accordance rate of 82.6%;the positive number of CFP was 11 eyes,with positive accordance rate of 47.8%.Progressive cataract is considered when the OSI value is between 4.0 and 7.0,which contains 200 eyes(retinal hemorrhage was actually diagnosed in 35 eyes after surgery).The preoperatively positive number of SLO was 20 eyes,with positive accordance rate of 57.1%;the positive number of CFP was 12 eyes,with positive accordance rate of 34.3%.Mature cataract is considered when the OSI value is greater than 7.0,which contains 60 eye
