摘要
目的 研究前臂静脉皮瓣修复手部软组织缺损的临床疗效.方法 回顾性分析2015年6月~2016年6月我院收治的手部软组织缺损患者62例,所有患者均使用前臂静脉皮瓣修复手术进行治疗,研究患者术后是否出现并发症、皮瓣成活质量、治疗满意率及SAS、SDS评分.结果 本组术后无并发症12例,且皮瓣成活质量较高.45例皮瓣出现肿胀(轻、中等程度),5例皮瓣术后出现严重水肿.本组患者术后手功能优32例,良16例,可9例,差5例.患者满意36例、较满意22例,不满意4例,患者满意率93.55%(58/62).与治疗前比较,治疗后患者的生活质量评分明显升高,差异有统计学意义(P<0.05);与治疗前比较,SAS、SDS评分显著降低,差异有统计学意义(P<0.05).结论 对手部软组织缺损患者使用前臂静脉皮瓣修复术进行治疗,能有效避免许多不良反应的发生,提高患者的生活质量及治疗效果,消除患者恐惧、不安的心理情绪,使其早日恢复健康,降低术后并发症发生率,值得进一步推广使用.
Objective To study the clinical effect of forearm free venous flap in repairing soft tissue defect.Methods 62 patients with soft tissue defects in hand admitted to our hospital from June 2015 to June 2016 were retrospectively analyzed.All patients were treated with forearm venous flap repair.Complications developed or not,flap survival quality,treatment satisfaction rate and self-rating anxiety scale(SAS)and self-rating depression scale(SDS)scores were studied.Results There were 12 patients with no complications in this group,and the survival quality of the flap was high.45 skin flaps were swollen(mild,moderate),and 5 skin flaps had severe edema after surgery.There were 32 cases being excellent,16 cases being good,9 cases being ok and 5 cases being poor.36 patients were satisfied,22 slightly satisfied,and 4 unsatisfied and the patient satisfaction rate was 93.55%(58/62).Compared with before treatment,the quality of life score after treatment significantly increased,and the difference was statistically significant(P<0.05);the SAS and SDS scores decreased,and the differences were statistically significant(P<0.05).Conclusion The application of forearm vein flap repair for soft tissue defects in hand can effectively avoid the occurrence of adverse reactions,improve the quality of life and treatment effect,eliminate the fear and anxiety of patients,restore their health and reduce the incidence of postoperative complications.It is worthy of further promotion and use.
