摘要
目的:探讨帕瑞昔布相关血小板减少、贫血及肝肾损害的发生率及影响因素。方法:采用回顾性研究方法,借助"医疗机构ADE主动监测与智能评估警示系统(ADE-ASAS)",对我院2018年1月1日至2018年12月31日期间使用帕瑞昔布的住院患者进行血小板减少、贫血及肝肾损害自动监测,报警病例经双人盲评确定阳性病例及各ADR发生率,并分析帕瑞昔布相关不良反应的影响因素。结果:共计纳入9540例帕瑞昔布用药患者,四个模块自动监测用时约45 min,经临床药师再评估,血小板减少、贫血、肝损害及肾损害的ADR发生率分别为0.65%、8.25%、1.01%、1.66%。帕瑞昔布致贫血在性别、年龄上差异有统计学意义,致肾损害在年龄上差异有统计学意义。结论:帕瑞昔布相关贫血发生率为常见,血小板减少、肝肾损害发生率均为偶见。自动监测能够高效、快速获得大样本目标人群的用药风险信息,并确定其相关影响因素,有助于降低临床用药风险。
Objective:To explore the incidence and the related factors of thrombocytopenia,anemia,liver and kidney injury induced by parecoxib.Methods:Data of patients in our hospital who received parecoxib from January 1st 2018 to December 31st 2018 were retrospectively extracted,and adverse drug reaction(ADR)of thrombocytopenia,anemia,liver and kidney injury induced by parecoxib were automatically monitored through the adverse drug events active surveillance and assessment system(ADE-ASAS).The incidence of ADR and the alarm cases were evaluated by two pharmacists.Related factors of ADR were simultaneously analyzed in this study.Results:A total of 9540 cases using parecoxib were enrolled in this study.It spent 45 minutes automatically monitoring four modules of thrombocytopenia,anemia,liver injury and kidney injury.After manual reevaluation of the clinical pharmacists,the incidence rate of thrombocytopenia,anemia,liver and kidney injury was respectively 0.65%,8.25%,1.01%and 1.66%.Sex and age were significantly associated with anemia caused by parecoxib,and age was significantly associated with kidney injury caused by parecoxib.Conclusion:Anemia caused by parecoxib was common,and thrombocytopenia,liver and kidney injury caused by parecoxib were incidental.ADE-ASAS could quickly obtained medication risk information of the large target population with high efficacy and be available for determining relevant factors,which could be helpful for reducing the risk of clinical medication.
