摘要
目的分析二甲双胍治疗对肥胖型多囊卵巢综合征(PCOS)患者妊娠率及妊娠期糖尿病发病率的影响。方法选取我院2017年7月—2018年12月收治的78例肥胖型PCOS患者,按治疗方案的不同分为以炔雌醇环丙孕酮片治疗的对照组、在对照组基础上加用二甲双胍治疗的观察组,每组39例。对比2组性激素指标、治疗效果、妊娠率及妊娠期糖尿病发病率。结果观察组FSH、T、LH等性激素水平指标明显低于对照组(P<0.05);观察组总有效率为94.87%,高于对照组的79.49%(P<0.05);观察组总怀孕率为82.05%,自然怀孕率为48.72%,高于对照组的58.97%和20.51%(P<0.05);观察组妊娠期糖尿病发病率为17.95%,低于对照组的25.64%,但差异无统计学意义(P>0.05)。结论炔雌醇环丙孕酮片联合二甲双胍可有效改善肥胖型PCOS患者月经周期不规律、多毛及顽固性痤疮,降低性激素水平,促进排卵,提高妊娠率。
Objective To analyze the effect of metformin treatment on the pregnancy rate of obese PCOS and the incidence of gestational diabetes mellitus.Methods 78 obese patients with PCOS admitted to our hospital from July 2017 to December 2018 were divided into two groups according to different treatment plans:the control group treated with ethinylestradiol and cyproterone tablets and the observation group treated with metformin on the basis of the control group,39 cases in each group.The pregnancy rate and the incidence of gestational diabetes were compared.Results the levels of FSH,t,LH and in the observation group were significantly lower than those in the control group(P<0.05);The total effective rate of the observation group was 94.87%,higher than that of the control group 79.49%(P<0.05);the total pregnancy rate of the observation group was 82.05%,the natural pregnancy rate was 48.72%,higher than 58.97%and 20.51%of the control group(P<0.05);the incidence of gestational diabetes was 17.95%,lower than 25.64%of the control group,but the difference was not statistically significant(P>0.05).Conclusion Ethinylestradiol and cyproterone combined with metformin can effectively improve irregular menstrual cycle,hirsute and intractable acne in obese PCOS patients,reduce sex hormone level,promote ovulation and improve pregnancy rate.
