摘要
目的对比石膏外固定与切开复位内固定治疗踝关节骨折的临床效果。方法选择我院2016年6月至2019年6月收治的70例踝关节骨折患者,随机分为对照组和试验组各35例。试验组行切开复位内固定治疗,对照组行石膏外固定治疗,比较两组的临床疗效、围术期相关临床指标、踝关节功能改善情况及并发症发生情况。结果试验组的临床治疗优良率为91.43%,高于对照组的71.43%(P <0.05)。试验组的住院时间长于对照组,骨折愈合时间短于对照组(P <0.05)。治疗后,两组的AOFAS评分高于治疗前,且试验组的AOFAS评分高于对照组(P <0.05)。两组的并发症发生率比较无统计学差异(P>0.05)。结论石膏外固定与切开复位内固定治疗踝关节骨折均有显著效果,且安全性较高。切开复位内固定的疗效更为显著,患者恢复更快,踝关节功能也恢复更好,而石膏外固定患者住院时间较短,因此应根据患者的实际情况选择合适的治疗方式。
Objective To compare the clinical effects of plaster external fixation and open reduction internal fixation in the treatment of ankle fracture.Methods 70 patients with ankle fracture admitted to our hospital from June 2016 to June 2019 were selected and randomly divided into control group and experimental group,with 35 cases in each group.The experimental group was treated with open reduction internal fixation,while the control group was treated with plaster external fixation.The clinical efficacy,perioperative related clinical indicators,ankle function improvemnet and complications of the two groups were compared.Results After treatment,the excellent and good rate of clinical treatment in the experimental group was 91.43%,higher than 71.43%in the control group(P<0.05).The hospitalization time of the experimental group was longer than that of the control group,and the fracture healing time was shorter than that of the control group(P<0.05).After treatment,the AOFAS scores in both groups were higher than those before treatment,and the AOFAS score of the experimental group was significantly higher than that of the control group(P<0.05).No statistical difference was found in the incidence of complications between the two groups(P>0.05).Conclusions Plaster external fixation and open reduction internal fixation in the treatment of ankle fracture have significant efficacy and higher safety.Open reduction internal fixation has more significant effect,faster recovery and better ankle function recovery,while plaster external fixation has shorter hospitalization time.Therefore,appropriate treatment methods should be selected according to patients'actual situation.
