摘要
目的:对针灸治疗炎症性肠病(IBD)的主穴进行分析,总结出普遍适用于IBD的穴位。方法:检索2007年9月1日-2018年9月1日中国期刊全文数据库、维普数据库、万方数据库、PubMed中的针灸治疗IBD的临床研究和临床报道文献,提取文献中选用的十四经腧穴,建立IBD的针灸临床处方数据库。运用SPSS 22.0和Clementine 12.0软件对数据进行描述性分析、关联规则分析和聚类分析。结果:获取文献190篇,提取穴方244首,涉及的64个腧穴中,天枢、足三里、关元、上巨虚使用频次最高,胃经穴位选用最多。胸腹部穴位选用最多,选穴重视局部取穴与远道取穴相配合。特定穴中选用最多的是募穴,下合穴、背俞穴次之。关联规则显示,支持度最高的是足三里-天枢。聚类分析得到3个有效聚类群和5对核心穴组。结论:临床运用针灸治疗IBD时,均可选用天枢、足三里、上巨虚、关元、气海、中脘作为基本穴位处方。
Objective:To summarize the acupoints generally applicable to inflammatory bowel disease(IBD)by analyzing the main acupuncture points in the treatment of IBD.Methods:Literatures on clinical study and clinical report for IBD treated with acupuncture and moxibustion were retrieved in CNKI,CQVIP,WANGFANG DATA and PubMed from September 1,2007 to September 1,2018.Fourteen meridians acupoints used in literatures were extracted to establish the database of acupuncture prescriptions for IBD.SPSS 22.0 and Clementine 12.0 were employed for the descriptive analysis,association rules analysis and clustering analysis.Results:A total of 190 eligible articles including 244 acupoint prescriptions met inclusion criteria.Among the 64 acupoints involved,Tianshu(ST25),Zusanli(ST36),Guanyuan(RN4)and Shangjuxu(ST37)were in the top four acupoints in turn,while stomach meridian was the most frequently used meridian.The most commonly used acupoints were located on thorax and abdomen.The combination of the local acupoints with the distal ones was predominated.The specific acupoints most commonly used were front-mu points,lower confluent points and back-shu points.The association rules showed that Zusanli(ST36)and Tianshu(ST25)had the highest correlativity.The clustering analysis figured out the 3 effective clusters and 5 pairs of key pionts.Conclusion:When clinically using acupuncture and moxibustion to treat IBD,Tianshu(ST25),Zusanli(ST36),Shangjuxu(ST37),Guanyuan(RN4),Qihai(RN6),Zhongwan(RN12)can be selected as the basic acupoint prescription.
