布地奈德联合特布他林雾化吸入治疗小儿百日咳的效果及对患儿免疫功能的影响 被引量:13

Effects of budesonide combined with terbutaline inhalation in treatment of pertussis in children and its influence on immune function of patients

摘要 目的观察布地奈德联合特布他林雾化吸入治疗小儿百日咳的疗效及对患儿免疫功能的影响。方法选取2016年10月至2019年12月收治的百日咳患儿116例,随机分为对照组和观察组,每组58例。2组均给予对症处理措施,对照组给予干扰素联合特布他林雾化吸入,观察组给予布地奈德联合特布他林雾化吸入。疗程5 d。比较2组临床疗效、症状改善情况(痉咳次数、痉咳时间、肺部啰音消失时间)、住院时间、肺功能指标[呼吸频率(RR)、潮气量(VT)、吸呼比(TI/TE)、达峰容积比(VPEF/VE)、达峰时间比(TPTEF/TE)]、炎性指标[白细胞计数(WBC)、C-反应蛋白(CRP)]、T淋巴细胞亚群(CD+4、CD+8、CD+4/CD+8)、免疫球蛋白(IgA、IgG、IgM)。结果对照组、观察组总有效率分别为82.76%、96.55%。观察组总有效率明显高于对照组(P<0.05)。观察组痉咳次数明显少于对照组,痉咳时间、肺部啰音消失时间、住院时间明显短于对照组(P<0.05)。治疗前,2组RR、VT、TI/TE、VPEF/VE、TPTEF/TE、WBC、CRP、CD+4、CD+8、CD+4/CD+8、IgA、IgG、IgM比较差异无统计学意义(P>0.05);治疗后,2组RR、WBC、CRP、CD+8明显低于治疗前,VT、TI/TE、VPEF/VE、TPTEF/TE、CD+4、CD+4/CD+8、IgA、IgG、IgM明显高于治疗前(P<0.05),且观察组RR、WBC、CRP、CD+8低于对照组,VT、TI/TE、VPEF/VE、TPTEF/TE、CD+4、CD+4/CD+8、IgA、IgG、IgM高于对照组(P<0.05)。结论布地奈德联合特布他林雾化吸入治疗小儿百日咳疗效显著,能够有效缓解临床症状,并改善患儿肺功能和免疫功能,值得临床推广应用。 Objective To investigate therapeutic effects of budesonide combined with terbutaline inhalation on pertussis in children and its influence on immune function of patients.Methods A total of 116 children with pertussis who were admitted and treated in our hospital from October 2016 to Dectober 2019 were enrolled in the study.The patients were randomly divided into observation group(n=58)and control group(n=58).The patients in both groups received symptomatic support treatment,on that basis,the patients in control group were treated by interferon combined with terbutaline inhalation,however,the patients in observation group were treated by budesonide combined with terbutaline inhalation,with a treatment course of 5 days for both groups.After treatment,the clinical curative effects,the improvement status of clinical symptoms,length of hospital stay,pulmonary function indexes(RR,VT,TI/TE,VPEF/VE,TPTEF/TE),inflammatory indexes(WBC and CRP),T lymphocytes(CD+4,CD+8 and CD+4/CD+8)and immunoglobulin(IgA,IgG and IgM)were observed and compared between the two groups.Results The total effective rate in observation group was 96.55%,which was significantly higher than that(82.76%)in control group(P<0.05).The frequency of spasmodic cough in observation group was significantly fewer than that in control group,however,the spasmodic cough time,the disappearance time of wet rales in the lungs and the length of hospital stay in observation group were significantly shorter than those in control group(P<0.05).Before treatment,there were no significant differences in the levels of RR,VT,TI/TE,VPEF/VE,TPTEF/TE,WBC,CRP,CD+4,CD+8,CD+4/CD+8,IgA,IgG and IgM between the two groups(P>0.05).After the treatment,the levels of RR,WBC,CRP and CD+8 in observation group were significantly decreased,as compared with those in control group,however,the levels of VT,TI/TE,VPEF/VE,TPTEF/TE,CD+4,CD+4/CD+8,IgA,IgG and IgM in observation group were significantly higher than those in control group(P<0.05).Conclusion The budesonide combined with terbutaline in
