摘要
目的在超声引导下从乳根穴把曲氨奈德注射到乳腺炎肿块的病灶区域,并口服红花逍遥片,观察治疗乳腺炎肿块的效果及患者的预后。方法收集2016年5月至2019年5月就诊的乳腺炎肿块患者108例,按就诊序号随机分为观察组和对照组,每组54例。对照组在常规治疗(给予抗生素头孢呋辛3.0 g,2次/d,静脉滴注,同时配合乳房按摩、微波治疗)的基础上,同时给以从乳根穴注射曲氨奈德注射液40 mg至肿块区域,观察组配合超声引导下从乳根穴精准注射曲氨奈德注射液40 mg至病灶区域,2组均口服红花逍遥片治疗(每次4片,3次/d,饭前饭后均可,不影响哺乳)。观察患者治疗后的肿块缩小速度、肿块图像的炎性彩超指标改善情况、痊愈指标、住院时间、住院费用,并对其预后进行随访。结果观察组患者治疗后肿块缩小速度、肿块图像的炎性彩超指标改善情况、痊愈指标、住院时间和住院费用下降都明显优于对照组,差异有统计学意义(P<0.05);随访预后,复发的病例少于对照组(P<0.05)。结论超声引导下从乳根穴把曲氨奈德精准注射到乳腺炎肿块的病灶区域并联合红花逍遥片治疗乳腺炎肿块患者,相对于仅凭经验注射,能取得更加可信的效果,其肿块缩小速度、肿块图像的炎性彩超指标改善情况、痊愈指标、住院时间和住院费用都要优于对照组,并且预后更好,取得显著社会效益和经济效益,应该常规临床推广。
Objective To investigate the therapeutic effects and prognosis of ultrasound-guided acupoint injection of triamcinolone acetonide combined with Honghuaxiaoyao tablet in treatment of mastitis masses.Methods A total of 108 patients with mastitis mass who were admitted and treated in our hospital from May 2016 to May 2019 were enrolled in the study,who were randomly divided into observation group(n=54)and control group(n=54)according to their visiting serial numbers.The patients in control group,on the basis of routine treatment(giving antibiotic Cefuroxime 3.0g twice daily intravenous drip,breast massage and microwave therapy),were injected with triamcinolone acetonide 40mg from the rugen point to the tumor area under the guidance of ultrasound,and the patients in observation group were injected with triamcinolone acetonide 40mg from the rugen point to the tumor area under the guidance of ultrasound.The patients in both groups were treated by oral Honghuaxiaoyao tablets(4 tablets once,3 times a day,before and after meals,whithout influencing lactation).The rate of tumor reduction,the improvement of inflammatory color doppler ultrasound,healing indexes,hospitalization time,hospitalization expenses and the prognosis of patients were observed and compared between the two groups.Results The tumor shrinking speed,the improvement of inflammatory color Doppler ultrasound,the healing indexes,the length of staying in hospital and the cost of hospitalization in observation group were significantly better than those in control group(P<0.05).The recurrence rate was significantly lower than that in control group(P<0.05).Conclusion The injection of triamcinolone acetonide into the tumor area under the guidance of ultrasound combined with Honghuaxiaoyao tablets can obtain more reliable effects in treatment of mastitis masses,as compared with those by injection based on experience only,the former can imprive the inflammatory indexes by color Doppler ultrasound,improve the recovery indexes,decrease the length of staying in hospital
