摘要
目的系统评价中国社区获得性肺炎(CAP)指南和专家共识的方法学质量。方法计算机检索中国知网(CNKI)、维普中文期刊数据库(VIP)、万方数字化期刊全文数据库(WANFANG DATA)、中国生物医学文献数据库(CBM)、医脉通指南网及相关学会网站收集国内公开发布的CAP指南和专家共识。运用AGREE-China工具对纳入的指南和专家共识进行质量评价。结果共纳入9部CAP指南和专家共识,其中中医指南/共识4部,西医指南/共识5部。9部指南/共识各领域的平均得分率分别是"科学性/严谨性"40.8%、"有效性/安全性"19.6%、"经济性"34.8%、"可用性/可行性"70.1%、"利益冲突"68.9%。各领域得分较高、整体印象较好的3部指南是《中国成人社区获得性肺炎诊断和治疗指南(2016年版)》《社区获得性肺炎中医诊疗指南(2018修订版)》《中医药单用/联合抗生素治疗常见感染性疾病临床实践指南:社区获得性肺炎》。结论国内CAP指南和专家共识的方法学质量逐步提高,但在有效性/安全性、经济性领域有待进一步改进。
Objective To systematically evaluate the methodological quality of guidelines and expert consensus of community acquired pneumonia(CAP)in China.Methods China National Knowledge Infrastructure(CNKI),Weipu Chinese journal database(VIP),Wanfang digital journal full text database(WANFANG DATA),CBM,the Medical Pulse Guide Network,and the related association websites were retrieved by computer to collect domestically published CAP guidelines and expert consensus.AGREE-China tool was used to evaluate the quality of the included guidelines and expert consensus.Results A total of 9 CAP guidelines and expert consensus were included,4 guidelines for Chinese medicine/consensus,and 5 guidelines for Western medicine/consensus.The average scores of the 9 guides/consensus in areas"scientific/rigorous","effectiveness/safety","economic","availability/feasibility"and"interest conflict"were 40.8%,19.6%,34.8%,70.1%and 68.9%respectively.Three guidelines with higher scores and better overall impressions in all fields were Guidelines for the Diagnosis and Treatment of Acquired Pneumonia in Adults in China(2016 Edition),Guidelines for the TCM Treatment of Community Acquired Pneumonia(2018 Revision),and Clinical Practice Guidelines for Common Infectious Diseases Treated by TCM/Combined Antibiotics Community Acquired Pneumonia.Conclusion The methodological quality of domestic CAP guidelines and expert consensus has gradually improved,but further improvements are needed in the fields of effectiveness/safety and economy.
