摘要
目的构建3D打印腹腔镜抗反流手术干实验室模型,探讨其在外科医师手术训练中的作用。方法选取18位无手术经验的外科医师随机均分成3组,A组和C组分别采用基础硅胶模型和3D打印模型进行手术训练,B组先后采用上述两种模型进行手术训练。应用手术技能客观结构化评估(OSATS)和5分制李克特(Likert)量表分别对训练表现和模型效能进行评分。组间比较采用t检验。结果所有外科医师均完成了相应手术训练和模型评分。3D打印模型和基础硅胶模型的整体印象评分为(4.28±0.67)分比(3.61±0.61)分(t=3.129,P<0.05),仿真度评分为(4.22±0.81)分比(3.39±0.50)分(t=3.716,P<0.05),差异均有统计学意义;实用性评分为(4.33±0.69)分比(3.89±0.76)分(t=1.844,P<0.05),触觉评分为(4.06±0.64)分比(3.94±0.80)分,美学评分为(3.94±0.73)分比(3.27±0.57)分(t=3.057,P<0.05),差异均有统计学意义。3D打印模型的训练效果明显优于基础硅胶模型,训练后OSATS评分C组[(14.17±1.81)分]>B组[(11.00±1.76)分,t=3.074,P<0.05]>A组[(9.08±1.50)分,t=2.031,P<0.05],完成时间C组[(73.17±5.23) min]<B组[(79.83±6.27) min,t=1.999,P<0.05]<A组[(87.50±7.40) min,t=1.936,P<0.05],差异均有统计学意义。结论 3D打印腹腔镜抗反流手术训练模型较传统模型优势明显,有助于缩短学习曲线和评价手术资质。
Objective To establish 3D printed dry laboratory model of laparoscopic anti-reflux surgery,and to explore its role in the training.Methods Eighteen surgical residents with same inexperience were enrolled in the training,and divided into three groups equally.Group B participated in the training using basic silica gel models and 3D printed models subsequentially,and groups A and C were subjected to the training using basic silica gel models and 3D printed models respectively.The performance was evaluated by Objective Structured Assessment of Technical Skills(OSATS),and models were evaluated by Likert scale.Results All participants completed the model surgical procedures and model evaluation.The scores of 3D printed models and basic silica gel models evaluated by Likert scale were as follows:overall impression 4.28±0.67 vs.3.61±0.61(t=3.129,P<0.05),realism 4.22±0.81 vs.3.39±0.50(t=3.716,P<0.05),practicability 4.33±0.69 vs.3.89±0.76(t=1.844,P<0.05),handling 4.06±0.64 vs.3.94±0.80,aesthetic 3.94±0.73 vs.3.27±0.57(t=3.057,P<0.05).The OSATS scores of Group C,B and A after training were 14.17±1.81,11.00±1.76(t=3.074,P<0.05),9.08±1.50(t=2.031,P<0.05),andthe operative time was(73.17±5.23)min,(79.83±6.27)min(t=1.999,P<0.05),(87.50±7.40)min(t=1.936,P<0.05),with statistically significant differences between any two groups.Conclusion The validity of 3D printed laparoscopic anti-reflux surgery dry laboratory model has been proved,which can shorten learning curve and is expected for operative evaluation.
