摘要
目的探讨老年胃癌根治术患者应用七氟烷复合瑞芬太尼的麻醉效果。方法按照随机数表法将2017年4月至2019年4月于临沂市中医医院行胃癌根治术的100例老年患者分为对照组和观察组,每组50例。对照组患者应用丙泊酚联合瑞芬太尼麻醉,观察组患者应用七氟烷联合瑞芬太尼麻醉,观察并比较两组患者不同时间段血流动力学变化及麻醉后恢复情况。结果两组患者在T0、T1、T2、T3时间段内收缩压(SBP)、舒张压(DBP)、心率(HR)及血压心率积(RPP)水平比较,差异未见统计学意义(P>0.05);观察组患者麻醉后清醒时间[(6.62±2.13)min]短于对照组[(11.28±3.04)min],且拔管时间[(9.40±2.71)min]短于对照组[(14.65±3.22)min],差异有统计学意义(P<0.05)。结论对老年患者胃癌根治术中应用七氟烷复合瑞芬太尼麻醉,可有效稳定其血流动力学,缩短清醒时间及拔管时间,麻醉效果及安全性均较好。
Objective To analyze the anesthetic effect of sevoflurane combined with remifentanil in elderly patients undergoing radical gastrectomy.Methods According to the method of random number table,100 elderly patients who underwent radical gastrectomy in Chinese Medicine Hospital of Linyi from April 2017 to April 2019 were divided into control group and observation group,with 50 cases in each groups.Patients in control group were treated with propofol combined with remifentanil anesthesia,while patients in observation group were treated with sevoflurane combined with remifentanil anesthesia.Hemodynamic changes and postoperative recovery of patients in the two groups were observed and compared at different time periods.Results There was no significant difference in systolic blood pressure(SBP),diastolic blood pressure(DBP),heart rate(HR)and Blood pressure heart rate product(RPP)between the two groups in T0,T1,T2 and T3(P>0.05).The awake time[(6.62±2.13)min]after anesthesia in the observation group was shorter than that in control group[(11.28±3.04)min],and the extubation time[(9.40±2.71)min]was lower than that in the control group[(14.65±3.22)min],the difference was statistically significant(P<0.05).Conclusions Sevoflurane combined with remifentanil anesthesia can effectively stabilize the hemodynamics,shorten the awake time and extubation time,and has certain anesthetic effect and safety.
