摘要
目的探讨质谱-色谱技术在产儿蛋白质代谢中的应用效果及其临床意义。方法方便选择2016年5月-2018年5月广东省珠海市中西医结合医院早产儿50例作为对象,设为观察组;选择同期分娩的足月儿50例,设为对照组。两组胎儿娩出后均完成尿液标本的采集,采用尿素酶预处理-气相色谱-质谱法(UP-GC-MS)完成两组尿液中的赖氨酸、苯丙氨酸、鸟氨酸、组氨酸、延延胡索酸及苹果酸的水平测定。结果观察组早产儿质谱-色谱技术测定后赖氨酸含量为0.20 mg/mL、苯丙乙酸含量为0.02 mg/mL、组氨酸含量为1.23 mg/mL,明显低于对照组足月儿(P<0.05);观察组早产儿检验尿液鸟氨酸平均秩次为29.49显著低于对照组的平均秩次45.57(t=-4.887);观察组延胡索酸平均值为29.39,显著低于对照组的平均值45.57(t=8.524);观察组苹果酸平均值为29.98,显著低于对照组平均值45.57(t=-2.720),差异有统计学意义(P<0.05)。结论早产儿的蛋白质分解代谢水平较低,而足月儿的蛋白质分解代谢水平较高。尿代谢物的测定可用于评估临床工作中新生儿的代谢状态,以进一步指导临床实施理性营养支持计划。
Objective To explore the application effect and clinical significance of mass spectrometry-chromatography in protein metabolism of infants.Methods From May 2016 to May 2018,50 cases of premature infants in Zhuhai Integrated Hospital of Traditional Chinese Medicine and Western Medicine in Guangdong Province were convenient selected as the observation group;50 cases of term infants who delivered at the same time were selected as the control group.Uterine samples were collected after the fetuses were delivered in both groups.Urinease pretreatment-gas chromatography-mass spectrometry(UP-GC-MS)was used to complete the lysine,phenylalanine,and ornithine in the two groups,histidine,fumaric acid and malic acid.Results The lysine content of preterm infants in the observation group was 0.20 mg/mL,the content of phenylpropionic acid was 0.02 mg/mL,and the histidine content was 1.23 mg/mL(P<0.05);the average rank of urine ornithine in the observation group of preterm infants was 29.49,which was significantly lower than the average rank of the control group,45.57(t=-4.887);The average value was 45.57(t=8.524);the average malic acid in the observation group was 29.98,which was significantly lower than the average 45.57 in the control group(t=-2.720).The difference was statistically significant(P<0.05).Conclusion The level of protein catabolism in preterm infants is low,while the level of protein catabolism in term infants is high.The determination of urinary metabolites can be used to evaluate the metabolic status of newborns in clinical work to further guide the clinical implementation of rational nutrition support programs.
