摘要
目的:以DRG指标为基础综合评价医院临床科室医疗服务产出情况,促进各临床专科向着精细化、精准化发展。方法:通过熵权-TOPSIS法和北京市临床重点专科评价方法的计算公式围绕能力、效率、安全3个维度对2018年临床科室的产出结果进行分别分析,再将两者结果进行综合比较研究。结果:医院CMI值稳步提升,科室16、科室33和科室35在两种评价方法中均排前三位,科室42均排最末,可见两种方法得到的评价结果一致性较好。结论:以DRG指标为基础,通过加权TOPSIS法和北京市临床重点专科评价方法结合开展医院临床科室间的横向比较,其评价结果符合医院实际,能为临床科室的精准化发展提供数据参考。同时由于两种方法在评价过程中对各指标的权重要求不一致,今后在评价中若对相应指标的重要性有所倾斜时,应视具体情况进行评价方法的合适选择。
Objective:Based on the DRG indicators to comprehensively evaluate the medical service outputs in clinical departments of the hospital to promote clinical specialties'refinement and precision development.Methods:Entropy Weight TOPSIS method and the calculation formulas about key clinical specialists'evaluation methods in Beijing were applied to respectively analyse.the output results of inpatient department in 2018 around ability,efficiency,and safety dimensions,and the results of the two were thoroughly compared.Results:The CMI of the hospital had been steadily improved.Department 16,Department 33 and Department 35 were in the top three among the two evaluation methods,Department 42 was in the bottom.The evaluation results resulted from the two methods were in good agreement.Conclusion:The results from horizontal comparison on the hospital's clinical departments through joint Entropy Weight TOPSIS method and the calculation formulas about key clinical specialists'evaluation methods in Beijing based on the DRG indicators are in accordance with the hospital's actuality and can provide data references for the clinical departments'precise development.Also due to different weight requirements of the two methods in the evaluation process,appropriate selection of the evaluation methods should be carried out according to the specific situations when the significance of the corresponding indicators in the evaluation inclines in the future.
