摘要
目的调查初诊白血病患者对疾病进展的恐惧现状及其对治疗依从性的影响。方法选取2014年1月~2018年11月于天津市第四中心医院血液科初诊白血病的156例患者作为研究对象,采用中文版恐惧疾病进展简化量表(FoP-Q-SF)、汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD-24)及Morisky用药依从性量表(MMAS-8)对其进行调查,评估患者白血病进展恐惧得分、焦虑得分、抑郁得分及治疗依从性得分。结果本次研究对象以男性、中老年、初中及以下文化程度、已婚、医疗保险/公费付费、育有1个及以上子女为主;患者白血病进展恐惧总分为(31.67±5.84)分,焦虑总分为(8.14±2.83)分,抑郁总分为(9.54±1.73)分,治疗依从性总分为(5.35±2.19)分。白血病进展恐惧得分与焦虑、抑郁呈正相关(r=0.734、0.682,P<0.05);与治疗依从性总分呈负相关(r=-0.801,P<0.05)。结论医护人员在积极治疗白血病的同时,应关注患者的心理变化,综合评估并积极对患者的心理问题进行干预,帮助患者缓解进展恐惧心理及焦虑抑郁情绪,减轻痛苦,改善生活质量。
Objective To investigate the current status of newly diagnosed leukemia patients'fear of disease progression and its effect on treatment compliance.Methods 156 patients who were newly diagnosed with leukemia in the Hematology Department of the Fourth Central Hospital of Tianjin from January 2014 to November 2018 were selected as the research subjects,and the Chinese version of the Simplified Scale of Fear Disease Progression(FoP-Q-SF)and Hamilton Anxiety Scale were used(HAMA),Hamilton Depression Scale(HAMD-24)and Morisky Medication Compliance Scale(MMAS-8)were used to investigate the patients'evaluation of leukemia progression fear score,anxiety score,depression score and treatment compliance score.Results The subjects of this study are mainly male,middle-aged and elderly,junior high school and below,married,medical insurance/public payment,and have one or more children;the total score of patients with leukemia progress fear is(31.67±5.84),the total score of anxiety was(8.14±2.83),the depression total score was(9.54±1.73),and the treatment compliance score was(5.35±2.19).The leukemia progression fear score was positively correlated with anxiety and depression(r=0.734,0.682,P<0.05);it was negatively correlated with the total score of treatment compliance(r=-0.801,P<0.05).Conclusion While actively treating leukemia,medical staff should pay attention to the psychological changes of patients,comprehensively evaluate and actively intervene in the psychological problems of patients,help patients to relieve the fear of progress and anxiety and depression,reduce pain,and improve the quality of life.
