摘要
目的探讨特发性膜性肾病患者肾组织及血液中抗磷脂酶A2受体(PLA2R)抗体表达的意义。方法选择2017年9月~2019年7月在我院肾内科就诊的45例特发性膜性肾病患者作为观察组,另选同期45例非特发性膜性肾病患者作为参照组,45例体检者作为健康组,比较三组肾功能常规指标[血尿素氮(BUN)、血肌酐(CREA)、血尿酸(UA)、以及血清白蛋白(ALB)]、血液中PLA2R抗体及肾组织中PLA2R抗体表达阳性率。结果观察组和参照组BUN、UA、CREA、ALB比较,差异无统计学意义(P>0.05);观察组和健康组BUN、UA、CREA、ALB比较,差异有统计学意义(P<0.05);观察组PLA2R抗体在肾组织表达性率为68.89%,与参照组的13.33%比较,差异有统计学意义(P<0.05);观察组在Ⅱ期(53.57%)、Ⅲ期(87.50%)及Ⅳ期(100.00%)的PLA2R抗体表达阳性率比较,差异有统计学意义(P<0.05)。结论特发性膜性肾病患者的肾功能也会出现异常情况,无论是肾组织还是血液中的PLA2R抗体表达都较高,且肾脏病理类型越重PLA2R抗体表达阳性率越高。
Objective To explore the significance of anti-phospholipase A2 receptor antibody expression in renal tissue and blood of patients with idiopathic membranous nephropathy.Methods 45 patients with idiopathic membranous nephropathy who were treated in the Department of Nephrology of our hospital from September 2017 to July 2019 were selected as the observation group,and 45 patients with non-idiopathic membranous nephropathy during the same period were selected as the reference group,and 45 medical examiners As a healthy group,compare the three groups of routine renal function indicators[blood urea nitrogen(BUN),blood creatinine(CREA),blood uric acid(UA),and serum albumin(ALB)],anti-phospholipase A2 receptor(PLA2R)in blood positive rate of antibody and PLA2R antibody expression in kidney tissue.Results There was no statistically significant difference between the observation group and the reference group in BUN,UA,CREA,and ALB(P>0.05);there was a statistically significant difference between the observation group and the healthy group in BUN,UA,CREA,and ALB(P<0.05);The expression rate of PLA2R antibody in the observation group was 68.89%in the kidney tissue,compared with 13.33%in the reference group,the difference was statistically significant(P<0.05);the observation group was in phaseⅡ(53.57%),phaseⅢ(87.50%)and compared with the positive rate of PLA2R antibody expression in stageⅣ(100.00%),the difference was statistically significant(P<0.05).Conclusion The renal function of patients with idiopathic membranous nephropathy will also be abnormal.Both the kidney tissue and the blood have higher expression of PLA2R antibody,and the heavier the pathological type of kidney,the higher the positive rate of PLA2R antibody expression.
