摘要
目的:比较胰岛素泵与多次皮下注射胰岛素在初诊2型糖尿病患者短期强化治疗中的效果。方法:选取76例初诊2型糖尿病患者为研究对象,依据随机数字表法分为观察组与对照组各38例。对照组采用多次皮下注射胰岛素治疗,观察组采用胰岛素泵短期强化治疗,比较两组空腹血糖(FBG)、餐后2 h血糖(PBG)、糖化血红蛋白(HbA1c)、胰岛β细胞功能指数(HOMA-β)、胰岛素抵抗指数(HOMA-IR)水平及不良反应情况。结果:治疗后,观察组FBG、PBG、HbA1c、HOMA-IR水平低于对照组,差异均有统计学意义(P<0.05);治疗后,观察组HOMA-β水平高于对照组,差异有统计学意义(P<0.05);观察组不良反应总发生率为5.26%(2/38),低于对照组的23.68%(9/38),差异有统计学意义(P<0.05)。结论:胰岛素泵用于初诊2型糖尿病的短期强化治疗效果优于多次皮下注射胰岛素,可有效降低血糖水平,恢复胰岛β细胞功能,降低胰岛素抵抗,且不良反应较少。
Objective:To compare effects of insulin pump and multiple subcutaneous injections of insulin in newly diagnosed type 2 diabetes patients.Methods:76 patients with newly diagnosed type 2 diabetes were selected as the research objects,and divided into observation group and control group according to the random number table method,38 cases in each.The control group was treated with multiple subcutaneous injections of insulin,while the observation group was treated with short-term intensive insulin pump therapy.The levels of fasting blood glucose(FBG),2h postprandial blood glucose(PBG),glycated hemoglobin(HbA1c),isletβ-cell function index(HOMA-β),insulin resistance index(HOMAIR)and adverse reactions were compared between the two groups.Results:After the treatment,the levels of FBG,PBG,HbA1c and HOMA-IR in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After the treatment,the level of HOMA-βin the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).The total incidence of adverse reactions in the observation group was 5.26%(2/38),which was lower than the control group of 23.68%(9/38),and the difference was statistically significant(P<0.05).Conclusions:The short-term intensive insulin pump therapy is superior to multiple subcutaneous injections of insulin in the newly diagnosed type 2 diabetes patients.It can effectively reduce the blood glucose level and restore the isletβ-cell function and insulin resistance level with fewer adverse reactions.
