摘要
目的了解西双版纳州肺结核登记病例的流行特征,探讨肺结核登记病例的空间分布和形成热点区域的原因。方法利用西双版纳州结核网络专报系统中肺结核登记病例资料对2015年肺结核的登记病例进行三间分布的描述,用Arc GIS和Geo Da软件进行空间自相关分析。结果 2015年西双版纳州肺结核登记率为78.90/10万,男性登记率高于女性(103.11/100 000 vs 54.62/100 000,χ2=23.225,P <0.001)。发病人群主要集中在18~34岁组439人(56.64%),职业构成中主要以农民为主478人(61.68%),汉族登记病例数247人(占31.87%),少数民族主要以哈尼族233人(占30.06%)。全局莫兰指数为0.214,登记病例在空间分布上具有聚集性,病例热点区域主要在景洪市的中部城区以及与勐腊镇的乡镇接壤处。结论 2015年西双版纳州肺结核登记率仍在上升,为进一步防控结核病蔓延,应加强对热点区域、邻近老挝缅甸乡镇的监测和干预防止热点区域的扩大和跨境卫生问题,同时还应加强少数民族聚集区和农村地区的肺结核健康教育。
Objective To understand the epidemiological characteristics of registered tuberculosis cases in Xishuangbanna,and to explore the spatial distribution of registered tuberculosis cases and the reasons for hot spots.Methods The cases of tuberculosis registered in Xishuangbanna in 2015 were described and analyzed by spatial autocorrelation with ArcGIS and GeoDa software.Results In 2015,the registration rate of tuberculosis in Xishuangbanna was 78.90/100,000,and the rate of men was higher than that of women(103.11/100,000 vs 54.62/100,000,χ2=23.225,P<0.001).The majority of the patients were 439(56.64%)in the 18-34 age group.478(61.68%)were farmers,247(31.87%)were Han people,and 233(30.06%)were Hani minority.The global Moran index was 0.214,and registered cases showed spatial clustering distribution.The focus areas of cases were mainly in the central urban area of Jinghong City and the border with Mengla county Conclusion The registration rate of tuberculosis in Xishuangbanna Prefecture was rising in 2015.In order to further prevent and control the spread of tuberculosis,surveillance and intervention should be strengthened to prevent the expansion of hot spots and to deal with cross-border health problems.Meanwhile health education on tuberculosis should also be strengthened in both minority areas and rural areas.
