摘要
目的探讨microRNA-26a-5p(miR-26a-5p)在重症脑梗死患者中的表达与预后的关系。方法选取50例重症脑梗死患者、50例轻型急性脑梗死(ACI)患者,另选取健康体检者50例为对照组,分别测定3组人员血清中miR-26a-5p的相对表达水平,同时测定脑梗死患者血清中C反应蛋白(CRP)和S100钙结合蛋白β(S100β)含量。根据miR-26a-5p的相对表达水平将重症脑梗死患者分为miR-26a-5p高表达组和miR-26a-5p低表达组,分别比较两组患者性别、年龄、继发院内感染率、脑梗死再发率、APACHE-Ⅱ评分、NIHSS评分、改良Ranking量表评分以及30 d预后的关系。结果轻型ACI组和重症脑梗死组血清中miR-26a-5p相对表达水平均高于对照组(P<0.01),重症脑梗死组血清中miR-26a-5p相对表达水平高于轻型ACI组(P<0.01)。重症脑梗死组血清中CRP含量高于轻型ACI组(P<0.01),但S100β含量无差异(P>0.05)。相关性分析发现轻型ACI组和重症脑梗死组血清中miR-26a-5p表达水平与CRP含量呈正相关,相关系数r为0.651和0.731(P<0.01);但与S100β无明显相关性(P>0.05)。重症脑梗死患者miR-26a-5p高表达组继发院内感染率、脑梗死再发率、APACHE-Ⅱ评分、NIHSS评分均高于miR-26a-5p低表达组,而改良Ranking量表评分低于miR-26a-5p低表达组。重症脑梗死患者miR-26a-5p高表达组中位总体生存时间小于miR-26a-5p低表达组。结论在重症脑梗死患者中检测血清中miR-26a-5p相对表达水平对判断预后有重要临床意义。
Objective To investigate the relationship between the expression of microRNA-26a-5p(miR-26a-5p)and prognosis in patients with severe cerebral infarction.Method 50 patients with severe cerebral infarction,50 patients with mild acute cerebral infarction(ACI),and 50 healthy people as control group were selected,the relative expression levels of miR-26a-5p in serum of 3 groups were determined,respectively.Meanwhile,the serum C-reactive protein(CRP)and S100 calcium binding proteinβ(S100β)levels in patients with cerebral infarction were measured.According to the relative expression level of miR-26a-5p,patients with severe cerebral infarction were divided into miR-26a-5p high-expression group and miR-26a-5p low-expression group.The relationship between gender,age,secondary nosocomial infection rate,recurrence rate of cerebral infarction,APACHE-II score,NIHSS score,modified Ranking scale score and 30-day prognosis was compared between the two groups,respectively.Results The relative expression level of miR-26a-5p in serum of mild ACI group and severe cerebral infarction group were both higher than that of control group(P<0.01).The relative expression level of miR-26a-5p in serum of severe cerebral infarction group was higher than that of mild ACI group(P<0.01).The serum CRP level in severe cerebral infarction group was higher than that in mild ACI group(P<0.01),but have no difference in serum S100βlevel(P>0.05).The correlation analysis showed that there was positive correlation between the expression levels of miR-26a-5p and CRP level in serum of mild ACI group and severe cerebral infarction group,the correlation coefficients r were 0.651 and 0.731(P<0.01),but have no significant correlation with S100βlevel(P>0.05).The secondary nosocomial infection rate,recurrence rate of cerebral infarction,APACHE-II score and NIHSS score in the miR-26a-5p high-expression group were higher than those in the miR-26a-5p low-expression group,while the modified Ranking scale score was lower than that in the miR-26a-5p low-expression gr
