摘要 目的探讨艾滋病病毒(HIV)感染与头发皮质醇浓度的关联性特点。方法选取200例HIV抗体阳性者为研究对象,20例HIV抗体阴性的健康者为对照组,其中研究对象中的20例作为病例组与对照组相配对,分析HIV感染者的性别、年龄、CD4+T淋巴细胞(简称CD4细胞)计数与头发皮质醇浓度水平的关系,同时分别比较两组的年龄、体质指数(BMI)、头发皮质醇浓度水平。结果 200例HIV感染者中,男性皮质醇浓度中位数为28.83(19.77~49.54)/pg/mg,女性为30.31(18.89~53.74)/pg/mg,两者差异无统计学意义(P> 0.05);<40岁头发皮质醇浓度中位数为24.26(16.97~41.24)/pg/mg,≥40岁为34.36(22.18~51.54)/pg/mg,两者差异有统计学意义(P <0.05);CD4细胞计数<350个/mm^3者皮质醇浓度中位数为36.48(22.60~50.94)/pg/mg,≥350个/mm^3者中位数为26.31(18.43~49.56)pg/mg,两者差异有统计学意义(P <0.05)。病例组与对照组比较,两组年龄、BMI中位数差异均无统计学意义(P> 0.05);病例组皮质醇浓度中位数为39.87(78.16~23.85)/pg/mg,对照组为21.61(14.83~26.75)/pg/mg,差异有统计学意义(P <0.05)。结论 HIV感染能促进皮质醇的分泌,不同性别间皮质醇浓度水平大致相同,而年龄越大、CD4细胞计数水平越低,皮质醇浓度水平越高。 Objective The current study aims to investigate the association between HIV infection and hair cortisol concentration level. Methods 200 HIV-positive patients were selected as study objects, and 20 HIV-negative patients were selected as the control group. 20 patients among the study objects as case group were matched with the control group, and relationship between gender, age, CD4^+ Tlymphocyte count and hair cortisol levels in HIV-infected patients were analyzed. Meanwhile, the age, body mass index(BMI), and cortisol levels in the two groups were compared. Results Among the 200 HIV-infected patients, the median cortisol concentration was 28.83(19.77-49.54)/pg/mg in men and 30.31(18.89-53.74)/pg/mg in women. There was no statistical difference between the two groups(P > 0.05). The median hair cortisol concentration of aged < 40 years old was 24.26(16.97-41.24)/pg/mg, and aged ≥ 40 years old was 34.36(22.18-51.54)/pg/mg, with the difference statistically significant between them(P < 0.05). The median cortisol concentration of CD4^+ T lymphocyte counts< 350/mm^3 was 36.48(22.60-50.94)/pg/mg, and that ≥ 350/mm^3 was 26.31(18.43-49.56)/pg/mg, with the difference between them statistically significant(P < 0.05). The differences of age and BMI between the case group and control group were not statistically significant(P > 0.05). The median cortisol concentration in the case group was 39.87(78.16-23.85)/pg/mg and the control group 21.61(14.83-26.75)/pg/mg, with the difference between them statistically significant(P < 0.05). Conclusion HIV infection can promote the secretion of cortisol, and the levels of cortisol are roughly the same between different genders. The older the age, the lower the CD4^+ T lymphocyte count level, and the higher the cortisol level.
