静吸复合全麻在腹部外科手术后停药顺序对苏醒拔管时间的影响观察

Observation on the Effect of Off-Medication Sequence of Combined Intravenous and Inhalation Anesthesia After Abdominal Surgery on the Awakening and Extubation Time

摘要 目的观察静吸复合全麻在腹部外科手术后停药顺序对苏醒拔管时间的影响。方法回顾性分析2018年7月至2019年5月于我院行静吸复合全麻下腹部外科手术的92例患者的临床资料,根据停药顺序的不同均分为两组。对照组先停用丙泊酚,观察组先停用七氟醚。比较两组不同时间点的平均动脉压(MAP)、心率(HR),比较两组的恢复情况及不良反应。结果两组入室后(T0)、拔除气管导管时(T1)、拔除气管导管5 min (T2)、拔除气管导管10 min (T3)的MAP、 HR比较,差异无统计学意义(P>0.05)。两组的苏醒时间、自主呼吸时间、拔除气管导管时间以及不良反应发生率比较,差异无统计学意义(P>0.05)。结论静吸复合全麻应用于腹部外科手术中的效果显著,不同停药顺序对患者苏醒及拔管时间、血流动力学无明显影响。 Objective To observe the effect of off-medication sequence of combined intravenous and inhalation anesthesia after abdominal surgery on the awakening and extubation time.Methods The clinical data of 92 patients with abdominal surgery under combined intravenous and inhalation anesthesia in our hospital from July 2018 to May 2019 were analyzed retrospectively.According to the different off-medication sequence,all cases were divided into two groups,with 46 cases in each group.Propofol was first discontinued in the control group and sevoflurane was first discontinued in the observation group.The mean arterial pressure(MAP)and heart rate(HR)at different time points were compared between two groups,and the recovery and adverse reactions were compared between two groups.Results No significant difference was found between two groups in MAP and HR after admission(T0),at removing the tracheal tube(T1),at 5 min after removing the tracheal tube(T2),and at 10 min after removing the tracheal tube(T3),all P>0.05.No significant difference was found between two groups in the awakening time,spontaneous breathing time,tracheal catheter extubation time and incidence of adverse reactions(P>0.05).Conclusions Combined intravenous and inhalation anesthesia has significant effect applied in abdominal surgery.Different off-medication sequence has no significant effect on patients'awakening time,extubation time and hemodynamics.
