Investigation on the prevalence of hospital infection in a third-level first-class hospital in Hunan Province from 2014-2019

摘要 目的了解2014-2019年湖南省某三级甲等医院医院感染发生特点和变化趋势,为制定医院感染预防控制和管理措施提供依据。方法采用电子病历调查和床旁调查相结合的方法,对2014-2019年湖南省某三级甲等医院某一时点的医院感染情况进行调查,分析医院感染发生率、感染科室分布、感染部位、病原菌检出和抗菌药物使用及送检情况。结果2014-2019年共调查住院患者5815例,医院感染患者217例(231例次),医院感染现患率为3.23%~4.71%;中心ICU和神经外科医院感染发生率较高;感染部位以下呼吸道感染为主;医院感染病原菌以革兰阴性菌为主,铜绿假单胞菌是检出率最高的多重耐药菌,占36.96%;抗菌药物使用率为29.33%~41.94%,不同年份间使用率比较,差异有统计学意义(χ^2=33.22,P<0.01);治疗性抗菌药物平均使用率为74.74%,药物联用以一联用药为主;治疗药物送检率呈上升趋势,由31.77%上升至61.96%,差异有统计学意义(χ^2=212.69,P<0.01)。结论湖南省某三级甲等医院抗菌药物管理虽有所成效,但仍需进一步加强医院感染防控工作。 Objective To understand the characteristics and trends of hospital infection in a third-level first-class hospital in Hunan Province from 2014-2019,and to provide a basis for the formulation of hospital infection prevention and control measures.Methods A combination of electronic medical record survey and bedside survey was adopted to investigate the hospital infection situation at a certain point in a third-level first-class hospital in Hunan Province from 2014-2019,and the incidence of hospital infection,distribution of infection departments,infection sites,and pathogen detection and using of antibacterial drugs and inspection situation were analyzed.Results From 2014-2019,a total of 5815 inpatients were investigated,and 217 patients with hospital infections(231 case-times),and the prevalence of hospital infections was 3.23%-4.71%.The incidence o f hospital infection in ICU and neurosurgery was higher,lower respiratory tract infections were the main infection sites.Gram-negative bacteria were the main pathogens of hospital infections,and Pseudomonas aeruginosa was the multi-drug resistant bacteria with the highest detection rate,accounting for 36.96%.The usage rate of antibacterial drugs was 29.33%-41.94%,and the difference between different years was statistically significant(χ^2=33.22,P<0.01).The average usage rate of therapeutic antibacterial drugs was 74.74%,and the drug combination was mainly one medicine.The inspection rate of therapeutic drugs was on the rise,from31.77%to 61.96%,and the difference was statistically significant(χ^2=212.69,P<0.01).Conclusion Although the management of antibacterial drugs in a third-level first-class hospital of Hunan Province has been effective,it is necessary to further strengthen the work of prevention and control of hospital infection.
