摘要
目的利用TCD监测颅内动脉的血流动力学变化,探讨甘露醇对急性大脑半球大面积梗死患者脑血流量和脑灌注压的影响。方法选取经颅脑CT和MRI确诊为大脑半球大面积梗死的40例患者为观察对象,随机分为A、B组,每组20例,A组患者在入院后15 min内快速静滴20%的甘露醇125 mL,B组患者在入院后15 min内快速静滴20%的甘露醇250 mL。利用TCD进行相关指标评价。结果A组患者治疗30 min后Vm较治疗前明显升高,PI较治疗前下降;B组患者在治疗30 min后Vm较治疗前升高,PI较治疗明显下降,差异有统计学意义(P<0.05);A组Vm升高幅度明显大于B组,PI下降幅度明显低于B组(P<0.05)。结论125 mL甘露醇静滴对急性大脑半球大面积梗死患者脑血流量的改善效果优于250 mL甘露醇。
Objective Using TCD to monitor the hemodynamic changes of intracranial arteries,to explore the effect of mannitol on cerebral blood flow and cerebral perfusion pressure in patients with acute cerebral hemisphere infarction.Methods Forty patients who were admitted to our department of neurology from May 2016 to July 2017 and diagnosed with cerebral hemisphere infarction by CT and MRI were selected as observation objects.Patients were randomly divided into group A and B,with 20 patients in each group.Group A patients received a rapid intravenous drip of 20%mannitol 125mL within 15 minutes after admission;patients in group B received a rapid intravenous drip 20%mannitol 250mL within 15 minutes after admission.Use TCD to evaluate related indicators.Results In group A,after 30 minutes of 125mL mannitol treatment,Vm was significantly higher than before treatment,and PI was lower than before treatment.In group B,patients were treated with 250mL mannitol 30 minutes after treatment,Vm was higher than before treatment,PI was significantly decreased than before,the difference was statistically significant;and the increase in Vm in group A was significantly larger than that in group B,and the decrease in PI was significantly lower than that in group B(P<0.05).Conclusion The improvement of cerebral blood flow in the 125mL mannitol intravenous infusion group is significantly better than that in the 250mL mannitol treatment group.
