摘要
为观察炎症性肠病(IBD)在结肠镜下的特异性表现及诊断准确率,回顾性分析本院340例行结肠镜检查的IBD患者临床资料。结果显示,340例中经病理组织活检确诊为IBD 337例,其中溃疡性结肠炎(UC)325例,克罗恩病(CD)12例。误诊3例为急性感染性肠炎,诊断准确率为99.11%。UC绝大多数病变累及直肠和乙状结肠,表现为肠黏膜充血和表浅溃疡。CD病变呈跳跃式节段性分布,好发于回肠末段和回盲部,主要表现为息肉样增生及鹅卵石样改变。结果表明,结肠镜检查对IBD诊断准确率较高,临床通过肠镜下黏膜表现和病理组织活检可准确诊断UC和CD。
This study was to observe the specific finding of inflammatory bowel diseases(IBD)under coloscope,to investigate IBD’s diagnostic accuracy,authors retrospectively analysed the clinical data of 340 IBD patients inspected coloscopy in author’s hospital.As results,in the 340 cases 337 were confirmed IBD through pathological biopsy,including 325 of ulcerative colits(UC),12 of Crohn disease(CD);3 were misdiagnosed as acute infectious enteronitis,thus total diagnostic accuracy was up to 99.11%,Also,the overwhelming majority of UC lesion involved rectum and sigmoid showing mucosa engorgement and epidermic ulcer;CD showing segmental distribution,and more occuring at ileum terminal and ileocecal site,mainly expressed as polypoid hyperplasia and cobble-like change.Results show that the accuracy of coloscopy on IBD is higher,and under coloscope UC and CD could be accuracyly diagnosed through mucosa and pathological biopsy findings.
