摘要
目的:研究重组人骨形态发生蛋白骨修复材料联合切开复位内固定治疗跟骨骨折的效果。方法:选择80例跟骨骨折患者,随机分为两组。对照组单独使用切开复位内固定术,观察组在切开复位内固定术的基础上,联合使用rh-BMP骨修复材料治疗。术后3个月,比较两组手术治疗的优良率、Bolher角、Maryland足部评分、Gissane角以及并发症情况,并比较两组的骨折愈合时间和骨不连发生率。结果:术后3个月,观察组的治疗优良率为87.50%,明显高于对照组的60.00%(P<0.05);术后3个月,观察组患者的Maryland足部评分、Bolher角以及Gissane角均明显高于对照组(P<0.05);观察组患者的并发症发生率为10.00%,与对照组的12.50%相比差异无统计学意义(P>0.05);观察组患者的骨折愈合时间和骨不连发生率均明显低于对照组(P<0.05)。结论:重组人骨形态发生蛋白骨修复材料联合切开复位内固定治疗跟骨骨折效果显著,可明显改善Bolher角、Maryland足部评分和Gissane角,缩短骨折愈合时间,减少骨不连的发生,还不会增加术后的并发症。
Objective:To explore the effect of recombined human bone morphogenetic protein combined with open reduction and internal fixation in the treatment of calcaneal fracture.Methods:80 cases of patients with calcaneal fracture were selected and divided into two groups randomly.The control group was treated with open reduction and internal fixation alone,and the observation group was treated with rh-BMP on the basis of open reduction and internal fixation.Three months after operation,the excellent and good rate,Boher angle,Maryland foot score,Gissane angle,complications,fracture healing time and the incidence of nonunion were compared between the two groups.Results:Three months after the operation,the excellent and good rate in the observation group was 87.50%,significantly higher than 60.00%in the control group(P<0.05),and the Maryland foot score,Boher angle and Gissane angle in the observation group were significantly higher than those in the control group(P<0.05).The incidence of complications in the observation group was 10.00%,and that in the control group was 12.50%,there was no difference between the two groups(P>0.05).The fracture healing time and the incidence of nonunion in the observation group were significantly lower than those in the control group(P<0.05).Conclusion:Recombinant human bone morphogenetic protein combined with open reduction and internal fixation has satisfactory effect on calcaneus fracture patients,can significantly improve the Boher angle,Maryland foot score and Gissane angle of patients,shorten the fracture healing time,reduce the incidence of bone nonunion,without increase of the postoperative complications.
