摘要
目的:探讨CD40基因RS1535045单核苷酸多态性与冠心病(CHD)及其所致猝死的相关性。方法:选取贵州医科大学法医司法鉴定中心2017-2018年共330例尸检血液样本,对照组192例,CHD组138例,其中单纯CHD患者72例,CHD猝死62例。利用多重扩增及高通量测序技术得到基因分型,用T检验以及卡方检验确定对照组及病例组基线资料差异,建立二元logistic回归评估各个变量的风险值。结果:RS1535045位点TC和TT基因型在病例组中的频率明显高于正常对照组,在CHD猝死组中TC和TT基因型的频率也高于单纯CHD组,经过二元logistic回归调整各个危险因素后显示RS1535045-T等位基因是CHD及CHD猝死的危险因素。经过Gensini评分分层回归后Gensini评分≤10时,RS1535045-T等位基因仍是CHD猝死的危险因素,而Gensini评分>10时,则无统计学意义。结论:CD40的RS1535045-T等位基因是CHD发生的危险因素;同时RS1535045-T等位基因也是CHD猝死发生的危险因素,并且其作用在较低冠状动脉狭窄组中更加明显。
Objective:To investigate the relationship between CD40gene RS1535045single nucleotide polymorphism and CHD and the correlation of sudden death caused by CHD.Method:From 2017to 2018,330autopsy blood samples were collected from Forensic Judicial Expertise Center of Guizhou Medical University,192cases in the control group and 138cases in the CHD group including 72cases with simple CHD and 62sudden death of CHD.Multiplex PCR and sequencing were used to obtain genotyping.T-test and chi-square test were used to determine any statistical difference between control group and case group,and binary Logistic Analysis was established to evaluate the risk values of each variable.Result:The frequency of TC and TT genotypes at RS1535045locus was significantly higher in the case group than in the control group.The frequency of TC and TT genotypes in the sudden death group was also higher than that in the simple CHD group.After adjusting the risk factors by binary logistic regression,it was found that RS1535045-T allele was a risk factor for sudden death of CHD and the sudden death caused by CHD.After hierarchical regression Gensini Score,When Gensini score≤10,RS1535045-T allele were still risk factors for sudden death of CHD.But when Gensini score>10,there was no significant difference.Conclusion:The RS1535045-T allele of CD40is a risk factor for CHD.The RS1535045-T allele is also a risk factor for sudden death of CHD,and its role is more obvious in the lower coronary artery stenosis group.
