关节镜下缝线“8”字缝合固定青少年胫骨髁间嵴撕脱骨折临床疗效观察 被引量:2

Clinical Observation of Arthroscopic and 8 Shape Suture Fixation for Treating Adolescent Tibial Eminence Avulsion Fracture

摘要 目的:探讨在关节镜辅助下复位,双股2号Ethibond(爱惜帮)缝线"8"字缝合固定青少年胫骨前交叉韧带止点髁间嵴撕脱骨折的临床疗效。方法:收集青少年前交叉韧带胫骨止点髁间嵴撕脱骨折患者14例。术中关节镜探查明确诊断,复位骨折块。双股2号Ethibond(爱惜帮)缝线由前交叉韧带腱骨止点处后方穿过,于韧带前方"8"字交叉,通过胫骨平台打双隧道引出缝线打结固定。术后随访12个月,术后疗效评价采用前抽屉试验、Lachman试验评价膝关节稳定性,Lysholm评分评价膝关节功能,术后3、6个月复查膝关节X线片评价骨折愈合情况。结果:术后膝关节X线片检查骨折块解剖复位,6个月时骨折均达骨性愈合。术后1年内(3、6、12个月)复查患膝前抽屉试验、Lachman试验均呈阴性。Lysholm评分由伤后术前(51.52±7.33)分恢复至末次随访12个月时(91.36±5.61)分,差异有统计学意义(P<0.05)。结论:在关节镜辅助下双股2号Ethibond(爱惜帮)缝线"8"字固定青少年前交叉韧带止点胫骨髁间嵴撕脱骨折,具有创伤小,骨折固定、愈合可靠,术后关节功能恢复良好的优点。 Objective:To observe the clinical effect of arthroscopic reduction and double-strand No.2 Ethibond suture“8”suture fixation of avulsion fracture of intercondylar crest at the anterior cruciate ligament of adolescents.Methods:A total of 14 cases of adolescent patients with avulsion fractures of anterior cruciate ligament and tibial inter-condylar crest were collected.Intraoperative arthroscopic exploration confirmed the diagnosis and reduced the fracture.The double-strand No.2 Ethibond suture passed through the anterior cruciate ligament tendon bone back and crossed the“8”in front of the ligament.The suture was tied through the double tunnel through the tibial plateau to fix the knot.The follow-up was performed for 12 months.The efficacy of the front drawer test and the Lachman test were used to evaluate the stability of the knee joint.The Lysholm score was used to evaluate the function of the knee joint.The knee joint radiographs were reviewed 3 and 6 months after the operation to evaluate the fracture healing.Results:Knee joint X-rays were used to check the anatomical reduction of the fracture block after operation.At 6 months,the fracture reached bone healing.Within 1 year(3,6,and 12 months)after operation,the anterior knee drawer test and Lachman test were negative.Lysholm score recovered from(51.52±7.33)points before injury to(91.36±5.61)points at the last follow-up 12 months,the difference was statistically significant(P<0.05).Conclusion:Arthroscopic assisted double-strand No.2 Ethibond(Ai Xibang)suture“8”fixed anterior cruciate ligament abutment tibial intercondylar ridge avulsion fracture of adolescents have small trauma,reliable fracture fixation,healing,and postoperative joint function recovery,which is worthy of clinical recommendation.
