摘要 目的:探讨奥氮平对精神分裂症(SP)患者认知功能和神经功能缺损评分的影响。方法:选取54例确诊为SP的患者,服用奥氮平治疗8周。统计患者治疗前后阳性与阴性症状量表(PANSS)、韦氏成人智力表(WAISR)、新韦氏记忆量表(WMSRC)、标准化精神分裂症认知功能成套测验(MCCB)、神经功能缺损评分量表(NIHSS)评分。采用酶联免疫吸附法测定患者治疗前后血清神经营养因子(NT-3)、白细胞介素(IL-6、IL-1β)以及脑源性神经营养因子(BDNF)水平。结果:治疗后PANSS评分较治疗前均显著下降(P<0.05);WAISR、WMSRC、MCCB评分均高于治疗前(P<0.05);NIHSS评分(8.46±2.40)分明显低于治疗前(12.18±3.12)分;NT-3、BDNF水平[(138.32±28.87)vs(115.28±25.27) ng·L^-1、(27.34±3.93)vs(21.06±3.65) ng·mL^-1]高于治疗前,IL-6、IL-1β水平[(11.81±1.43)vs(14.67±1.89) pg·L^-1、(15.98±3.27)vs(24.50±3.78) pg·L^-1]均低于治疗前(P<0.05)。结论:奥氮平能有效提高精神分裂症患者认知功能,改善神经缺损水平。 Objective:To investigate the effects of olanzapine on cognitive function and neurological deficit score in patients with schizophrenia(SP).Methods:A total of 54 patients diagnosed with SP were selected and treated with olanzapine for a course of 8 weeks.The scores of positive and negative symptom scales(PANSS),Weiss Adult Intelligence Scale(WAISR),Chinese Wechsler Memory Scale-Revised(WMSRC),MATRICS consensus cognitive battery(MCCB),National institute of health stroke sgale(NIHSS)were statistically analyzed.The levels of serum neurotrophin-3(NT-3),interleukin(IL-6,IL-1β)and brain-derived neurotrophic factor(BDNF)before and after treatment were measured by enzyme-linked immunosorbent assay.Results:After treatment,PANSS scores were significantly lower than those before treatment(P<0.05).After treatment,average values of WAISR,WMSRC and MCCB scores were increased(P<0.05).The NIHSS score(8.46±2.40)after treatment was significantly lower than that before(12.18±3.12);compared with before treatment,the levels of NT-3 and BDNF after treatment[(138.32±28.87)vs(115.28±25.27)ng·L^-1,(27.34±3.93)vs(21.06±3.65)ng·mL^-1]higher than before treatment,IL-6,IL-1βlevels[(11.81±1.43)vs(14.67±1.89)pg·L^-1,(15.98±3.27)vs(24.50±3.78)pg·L^-1]were lower than before treatment(P<0.05).Conclusion:Olanzapine can effectively improve cognitive function and improve the level of nerve deficit in SP patients.
