摘要
目的探讨胰岛素样生长因子-1(IGF-1)、空腹血糖(FPG)、甲胎蛋白(AFP)在原发性肝癌(PHCC)合并2型糖尿病患者血清中的改变及其临床意义。方法收集正常人群组、2型糖尿病组、PHCC组和PHCC合并2型糖尿病组人群基本资料及血液样本,检测外周血IGF-1、FPG、AFP水平的变化;并对PHCC合并2型糖尿病组患者治疗前后血清IGF-1、AFP水平变化进行分析。结果与正常人群组、2型糖尿病组及PHCC组相比,PHCC合并2型糖尿病组患者血清IGF-1水平降低,差异有统计学意义(P<0.05),且期别越晚IGF-1水平越低。与正常人群组相比,PHCC组、PHCC合并2型糖尿病组患者血清FPG、AFP水平均升高,差异有统计学意义(P<0.05)。与同期治疗前相比,治疗后PHCC合并2型糖尿病组患者外周血清IGF-1水平升高,AFP水平降低,差异有统计学意义(P<0.05)。结论PHCC患者血清IGF-1水平低于正常人群,PHCC合并2型糖尿病组患者血清IGF-1水平低于正常人群及2型糖尿病组患者,IGF-1可作为2型糖尿病患者肝癌发生的预测指标。
Objective To investigate changes in insulin-like growth factor-1(IGF-1),fasting plasma glucose(FPG),primary hepatocellular carcinoma(PHCC),a-fetoprotein(AFP)in patients with PHCC and type 2 diabetes mellitus(T2DM)and its effect on prognosis.Methods The basic data and blood samples of normal group,T2DM group,PHCC group and PHCC combined with T2DM group were collected to detect the changes of IGF-1,FPG and AFP,and changes of IGF-1 and AFP in serum before and after treatment were analyzed.Results Compared with the normal group,the T2DM group and PHCC group,the level of IGF-1 in the patients with PHCC and T2DM group decreased significantly(P<0.05),and the later the period,the lower the level of IGF-1.Compared with the normal group,the level of FPG and AFP in the PHCC group and PHCC combined with T2DM group decreased significantly(P<0.05),Compared with before the treatment,the level of IGF-1 and AFP in the patients with PHCC and T2DM increased significantly(P<0.05).Conclusion The serum IGF-1 level of the patients with PHCC is lower than that of the normal people,and the serum IGF-1 level of the patients with PHCC and T2DM is lower than that of the normal people and T2DM.IGF-1 can be used as a predictive index for the occurrence of liver cancer in the patients with T2DM.
