摘要
目的观察扶肺固肾膏联合西医常规治疗肺肾两虚型慢性阻塞性肺疾病(COPD)稳定期临床疗效及对患者免疫功能的影响。方法将80例肺肾两虚型COPD稳定期患者按照随机数字表法分为2组。对照组40例(脱落1例)予沙美特罗替卡松粉吸入剂治疗;治疗组40例(脱落1例)在对照组治疗基础上加用扶肺固肾膏治疗。2组均治疗3个月后统计临床疗效,观察2组治疗前后COPD评估测试(CAT)评分、第1 s用力呼气容积占预计值百分比(FEV 1%),血清CD3^+、CD4^+、CD8^+、CD4^+/CD8^+,血清免疫球蛋白A(IgA)、IgM、IgG及痰分泌性免疫球蛋白A(sIgA)情况。结果治疗组总有效率92.31%(36/39),对照组总有效率76.92%(30/39),治疗组临床疗效优于对照组(P<0.05)。治疗后2组CAT评分均降低(P<0.05),且治疗组低于对照组(P<0.05)。治疗后2组FEV 1%与本组治疗前及组间比较差异均无统计学意义(P>0.05)。治疗后治疗组血清IgA、IgM、IgG、CD3^+、CD4^+、CD4^+/CD8^+及痰sIgA水平较本组治疗前均升高(P<0.05),血清CD8^+较本组治疗前降低(P<0.05),且治疗后治疗组血清IgA、IgM、IgG、CD3^+、CD4^+、CD4^+/CD8^+及痰sIgA水平均高于对照组(P<0.05),血清CD8^+低于对照组(P<0.05)。对照组治疗前后IgA、IgM、IgG、CD3^+、CD4^+、CD8^+、CD4^+/CD8^+及痰sIgA比较差异无统计学意义(P>0.05)。结论扶肺固肾膏联合西医常规治疗肺肾两虚型COPD稳定期疗效确切,能改善生活质量,提高免疫功能。
Objective To observe the effect of Fufei Gushen plaster combined with western medicine on chronic obstructive pulmonary disease(COPD)of lung and kidney deficiency type and its influence on the immune function of patients.Methods 80 cases of COPD with lung and kidney deficiency type were divided into two groups according to random number table method.40 cases(with one case dropped)in control group were treated by salmeterol ticasone powder inhalation.40 cases(with one case dropped)in treatment group were treated by Fufei Gushen plaster on the basis of the control group.The clinical effects of the two groups were statistically analyzed after 3 months of treatment.Before and after treatment,the COPD evaluation test(CAT)score,the forced expiratory volume in the first second as a percentage of the expected value(FEV 1%),serum CD3^+,CD4^+,CD8^+,CD4^+/CD8^+,serum immunoglobulin A(IgA),IgM,IgG and sputum secretory immunoglobulin A(sIgA)were observed.Results The total effective rate was 92.31%(36/39)in the treatment group and 76.92%(30/39)in the control group.The clinical effect of the treatment group was better than that of the control group(P<0.05).After treatment,CAT scores of the two groups were all decreased(P<0.05),and the treatment group was lower than the control group(P<0.05).There was no significant difference in FEV 1%between the two groups before and after treatment(P>0.05).After treatment,the levels of serum IgA,IgM,IgG,serum CD3^+,CD4^+,CD4^+/CD8^+and sputum sIgA in the treatment group were increased(P<0.05),while serum CD8^+was decreased(P<0.05).After treatment,the levels of serum IgA,IgM,IgG,serum CD3^+,CD4^+,CD4^+/CD8^+and sputum sIgA in the treatment group were higher than those in the control group(P<0.05),and serum CD8^+were lower than those in the control group(P<0.05).There was no significant difference in IgA,IgM,IgG serumCD3^+,CD4^+,CD8^+,CD4^+/CD8^+and sputum SIgA before and after treatment in the control group(P>0.05).Conclusion Fufei Gushen plaster combined with western medicine is effective in
