摘要
目的观察维格列汀联合二甲双胍治疗对糖尿病合并肾损伤患者的治疗效果及对患者肾功能和γ-谷氨酰转移酶(GGT)的影响。方法选取2016年2月至2018年8月接受治疗的糖尿病合并肾功能损伤患者80例,随机分为对照组和观察组,每组40例。2组患者均给予二甲双胍治疗,观察组在此基础上给予维格列汀治疗。观察2组患者治疗前后血糖情况、肾功能指标、炎性因子和不良反应发生情况。结果2组患者治疗前血糖差异无统计学意义(P>0.05),治疗后,观察组患者的FBG、2 h PG和HbA1c水平低于对照组(P<0.01);2组患者治疗前肾功能指标差异无统计学意义(P>0.05),治疗后,观察组患者的BUN、SCr和GCT水平低于对照组,GFR水平高于对照组(P<0.05);2组患者治疗前炎性因子水平差异无统计学意义(P>0.05),治疗后,观察组患者的CRP、TNF-α、IL-6水平低于对照组患者(P<0.05);2组患者咳嗽、恶心和头晕发生率差异无统计学意义(P=0.644)。结论维格列汀联合二甲双胍治疗对糖尿病合并肾损伤患者的治疗效果较好,可明显改善患者的肾功能,具有良好的应用价值。
Objective To investigate the effects of vildagliptin combined with metformin on the renal function andγ-glutamyltransferase(GGT)in patients with diabetes mellitus complicated by renal injury.Methods A total of 80 patients with diabetes mellitus complicated by renal injury were enrolled in the study,who were randomly divided into control group and observation group,with 40 cases in each group.The patients in both groups were treated by metformin,on that basis,the patients in observation group were treated by vildagliptin.The changes of blood glucose,renal function,inflammatory factors and the incidence of adverse reactions before and after treatment were observed and compared between the two groups.Results Before treatment there were no significant differences in blood glucose levels between the two groups(P>0.05).After treatment,the levels of FBG,2hPG and HbA1c in observation group were significantly lower than those in control group(P<0.01).Before treatment there were no significant differences in renal function parameters between the two groups(P>0.05).After treatment,the levels of BUN,SCr,GFR and GCT in observation group were significantly lower than those in control group,however,the GFR levels were significantly higher than those in control group(P<0.05).Before treatment there were no significant differences in inflammatory factors levels between the two groups(P>0.05).After treatment,the levels of CRP,TNF-α,and IL-6 in the observation group were significantly lower than those in control group(P<0.05).There were no significant differences in the incidence rates of cough,nausea,and dizziness between the two groups(P>0.05).Conclusion The combination application of vildagliptin with metformin is effective in treatment of diabetes mellitus complicated by renal injury,which can significantly improve the renal function of patients.
