摘要
目的探讨彩色多普勒超声诊断鳃裂囊肿的声像学特点。方法选取疑似鳃裂囊肿患者50例,所有患者均行彩色多普勒超声检查,随访记录病理检查结果,以病理检查结果为金标准,评估彩色多普勒超声诊断鳃裂囊肿的价值,并分析鳃裂囊肿行彩色多普勒超声检查的声像学特点。结果50例患者中,经手术病理检查,确诊48例患者为鳃裂囊肿,其中Ⅰ型鳃裂囊肿10例,占20.83%,Ⅱ型鳃裂囊肿34例,占70.84%,Ⅲ型鳃裂囊肿4例,占8.33%;经彩色多普勒超声检查44例诊断结果完全符合,误诊4例,诊断总符合率为91.67%,其中Ⅰ型鳃裂囊肿诊断符合率为80.00%、Ⅱ型鳃裂囊肿诊断符合率为97.06%、Ⅲ型鳃裂囊肿诊断符合率为75.00%;以病理检查结果为“金标准”,彩色多普勒超声检查鳃裂囊肿灵敏度为91.67%,特异度为50.00%,准确率为90.00%。彩色多普勒超声声像学特点显示,囊肿多呈类圆形或圆形,边界清晰,内部呈多样性回声,少数结节呈不规则形状,内部回声不均匀;彩色多普勒血流成像(color doppler flow imaging,CDFI)显示囊肿中均无明显血流信号,仅10例囊肿边缘显示少量呈条、点状血流信号。结论彩色多普勒超声可清晰显示鳃裂囊肿的形态、内部回声、结构及血液供应情况,可作为临床诊断鳃裂囊肿的首选方式,为临床治疗提供重要依据。
Objective To investigate the ultrasonographic characteristics of color Doppler ultrasound in the diagnosis of branchial cleft cyst.Methods 50 patients with suspected branchial cleft cyst were selected,all of whom were given color Doppler ultrasound examination.The results of pathological examination were recorded during follow-up.With the pathological examination results as the gold standard,the value of color Doppler ultrasound in the diagnosis of branchial cleft cyst was evaluated,and the ultrasonographic characteristics of color Doppler ultrasound in the diagnosis of branchial cleft cyst were analyzed.Results Among the 50 patients,48 cases were diagnosed with branchial cleft cyst by operative and pathological examination,including 10 cases(20.83%)of Type-Ⅰbranchial cleft cyst,34 cases(70.84%)of Type-Ⅱbranchial cleft cyst,4 cases(8.33%)of Type-Ⅲbranchial cleft cyst.According to the color Doppler ultrasound examination results,44 cases were completely consistent with the above diagnosis,4 cases were misdiagnosed,and the total diagnostic coincidence rate was 91.67%(Type-Ⅰbranchial cleft cyst 80.00%,Type-Ⅱbranchial cleft cyst 97.06%,Type-Ⅲbranchial cleft cyst 75.00%).With the pathological examination results as the gold standard,the sensitivity,specificity and accuracy of color Doppler ultrasound in the examination of branchial cleft cyst were respectively 91.67%,50.00%,90.00%.The ultrasonographic characteristics of color Doppler ultrasound showed that the cysts are mostly nearly-circular or round with clear boundary and diverse internal echoes,a few nodules were irregular in shape,and the internal echo was uneven.The color doppler flow imaging(CDFI)showed no obvious blood flow signal in the cyst,only 10 cases with a few strip or point blood flow signals at the cyst margin.Conclusion Color Doppler ultrasound can clearly show the shape,internal echo,structure and blood supply of branchial cleft cyst,which can be used as the first choice for the clinical diagnosis of branchial cleft cyst,and provide an i
