下颌第一前磨牙与桩核冠修复相关解剖因素的显微CT测量研究

摘要 目的:采用显微CT(Micro-CT)研究下颌第一前磨牙,测量其与桩核冠修复相关的解剖数据。方法:收集47例离体下颌第一前磨牙,采用显微CT扫描,区分I类与非I类根管,选取釉牙骨质界(cemento-enamel junction,CEJ)等多个平面,分别利用软件测量根径、根管径、根管壁厚度、根面沟等解剖数据,进行统计分析。结果:该研究中I类根管占比59.57%,与非I类根管牙齿外部形态解剖数据无统计学差异。距根尖4mm处近远中根管壁平均厚度分别为0.91mm、0.93mm,距离6mm时近远中壁平均厚度分别为1.03mm.1.07mm,但数值变异较大。根面沟影响根管形态,容易出现薄弱根管壁。结论:采用Micro-CT可准确重建牙体并进行相关数据测量。下颌第一前磨牙根管解剖结构复杂,根管桩长度尽量设置在距根尖4mm以上,尤其存在根面沟时应对薄弱侧壁更加谨慎。 Objective:To investigate the mandibular first premolar with Micro-CT,and measure the anatomical data related to post-and-core crown restoration.Methods:47 mandibular first premolars in vitro were collected and scanned.Microscopic CT scanning was used to distinguish class I and non-class I root canals.Several planes including the CEJ plane were selected as reference planes.The diameter of the root,root canal and root canal wall thickness as well as other anatomical data related to the groove were measured using the software and statistically analyzed.Results:In this study,the proportion of type I root canal was 59.57%.There was no statistical difference in external morphological data between teeth with class I and non-class I root canal.The average thicknesses of the mesial and distal root canal wall at 4 mm plane from the root tip were 0.91 mm and 0.93 mm respectively.When the distance was 6 mm,the average thicknesses of the mesial and distal root canal wall were 1.03 mm and 1.07 mm respectively,but the partial variation is large.The root groove affects the root canal shape,and the weak root canal wall is more likely to appear.Conclusion:The Micro-CT method can be used to accurately reconstruct the teeth and measure the data.The root canal morphology of the mandibular first premolar is complicated,and the length of the post should be set above the 4 mm plane from the root tips.Especially when the root surface groove exists,the weak lateral wall deserves more attention.
