摘要
目的分析生物反馈电刺激联合盆底肌肉训练对自然分娩初产妇产后早期盆底功能障碍性疾病的临床疗效。方法选择2018年1月至2019年5月上海市第七人民医院收治的98例自然分娩且产后发生早期盆底功能障碍性疾病的初产妇为研究对象,根据随机数字表法将其分为观察组和对照组,每组各49例。对照组患者接受盆底肌肉训练,观察组患者在对照组基础上联合生物反馈电刺激治疗。两组患者疗程均为8周。比较两组患者疗效、治疗前后Ⅰ类和Ⅱ类肌纤维肌力改善情况、女性性功能指数(female sexual function index,FSFI)评分和盆底功能影响问卷简表(pelvic floor impact questionnaire 7,PFIQ-7)评分。结果观察组患者治疗总有效率显著高于对照组(P<0.05)。两组患者治疗后Ⅰ类和Ⅱ类肌纤维肌力持续收缩压、FSFI评分和PFIQ-7评分均显著高于本组治疗前(均P<0.05),Ⅰ类和Ⅱ类肌纤维肌力收缩持续时间均显著长于本组治疗前(均P<0.05);观察组患者治疗后Ⅰ类和Ⅱ类肌纤维肌力持续收缩压、FSFI评分和PFIQ-7评分均显著高于对照组(均P<0.05),Ⅰ类和Ⅱ类肌纤维肌力收缩持续时间均显著长于对照组(均P<0.05)。结论生物反馈电刺激联合盆底肌肉训练可改善自然分娩且产后发生早期盆底功能障碍性疾病的初产妇的盆底肌力和盆底功能,提高其性生活质量。
Objective To investigate the effect of biofeedback electrical stimulation combined with pelvic floor muscle training on early pelvic floor dysfunction after parturition.Method Ninety-eight patients with early pelvic floor dysfunction after parturition admitted to Shanghai Seventh People's Hospital from January 2018 to May 2019 were randomly divided into observation group(49 cases)and control group(49 cases)according to the random number table method.Control group patients were treated with pelvic floor muscle training,and observation group patients were treated with biofeedback electrical stimulation on the basis of control group.The course of treatment in both groups was 8 weeks.The improvement of typeⅠand typeⅡpelvic floor muscle fiber,female sexual function index(FSFI)score and pelvic floor impact questionnaire 7(PFIQ-7)score were compared between the two groups before and after treatment.Result The total effective rate of observation group was significantly higher than that of control group(P<0.05).After treatment,the sustained contraction pressure of typeⅠand typeⅡpelvic floor muscle fiber,FSFI score and PFIQ-7 score in the two groups were significantly higher than those before treatment(all P<0.05),the duration of contraction of typeⅠand typeⅡpelvic floor muscle fiber were significantly longer than those before treatment(all P<0.05);the sustained contraction pressure of typeⅠand typeⅡpelvic floor muscle fiber,FSFI score and PFIQ-7 score in observation group were significantly higher than those of control group(all P<0.05),the duration of contraction of typeⅠand typeⅡpelvic floor muscle fiber were significantly longer than those of control group(all P<0.05).Conclusion Biofeedback electrical stimulation combined with pelvic floor muscle training can improve pelvic floor muscle strength and pelvic floor function of primiparas who have natural childbirth and early postpartum pelvic floor dysfunction diseases,and improve their sexual life quality.
