摘要
目的探讨不同类型急性阑尾炎诊断中超声检查的应用价值。方法选取本院于2018年5月至2019年5月收治的114例急性阑尾炎患者,依据手术病理划分为3组,即急性单纯性阑尾炎(A组,23例)、急性化脓性阑尾炎(B组,77例)与急性坏疽性阑尾炎(C组,14例),均开展超声检查,对比检查结果。结果在所选取患者当中,右下腹压痛113例,右下腹反跳痛103例,中性粒细胞比例>70%为101例,体温升高50例,白细胞计数>10×10^9/L为102例,超声见靶环征90例,盲端的指状低回声肿块102例。C组靶环征比例较其他两组偏低(P<0.05),B组阑尾周围积液比例与A组相比,显著偏高(P<0.05)。结论反跳痛、右下腹压痛,白细胞、中性粒细胞与体温升高,经超声检查出现盲端的指状低回声肿块与靶环征为急性阑尾炎的典型特点,而靶环征者多为化脓性阑尾炎或单纯性阑尾炎,阑尾周围积液则多为化脓性阑尾炎。
Objective To explore the value of ultrasonography in the diagnosis of different types of acute appendicitis.Methods 114 patients with acute appendicitis admitted to our hospital from May 2018 to May 2019 were divided into three groups according to the operation pathology:acute simple appendicitis(group A,23 cases),acute suppurative appendicitis(group B,77 cases)and acute gangrenous appendicitis(Group C,14 cases).The results of ultrasound examination were compared.Results Among the selected patients,113 cases had tenderness in the right lower abdomen,103 cases had rebound pain in the right lower abdomen,101 cases had neutrophil ratio greater than 70%,50 cases had temperature increase,102 cases had white blood cell count greater than 10×10^9/L,90 cases had target ring sign and 102 cases had finger-like hypoechoic mass in the blind end.The ratio of target ring sign in group C was lower than that in other two groups(P<0.05),and the ratio of periappendiceal effusion in group B was significantly higher than that in group A(P<0.05).Conclusion The typical features of acute appendicitis are rebound pain,right lower abdominal tenderness,elevated white blood cells,neutrophils and body temperature.The finger-like hypoechoic mass and target ring sign at the blind end appear by ultrasonography,while the target ring sign is mostly purulent appendicitis or simple appendicitis,while the periappendiceal effusion is mostly purulent appendicitis.
