摘要
目的分析社区老年高血压患者跌倒现状及干预效果。方法以130例60~79岁高血压患者为研究对象,采用调查表和体检了解跌倒状况,随机分为对照组和观察组,每组各65例,对照组予以常规社区卫生服务,观察组予以常规社区卫生服务+家庭访视,干预时间均为12个月。结果女性、年龄、动态平衡和患慢性病是跌倒的危险因素,而防跌行为和防跌倒效能则是保护因素;观察组防跌知识、行为、效能、静态平衡、姿势控制及动态平衡均显著优于对照组(P<0.01),年人均跌倒次数显著降低(P<0.01)。结论家庭访视能够改善患者平衡能力,提高防跌知信行水平和防跌倒效能,降低跌倒次数,值得推广应用。
OBJECTIVE To analyze the current status and influencing factors of falls in elderly hypertensive patients in the community,evaluate the effect of home visit interventions,and prevent and reduce falls.METHODS With the help of the health filing system of the community health service center,a random cluster of 130 hypertensive patients aged 60~79 years were selected as the research object,and a self-made questionnaire,fall knowledge and behavior,fall efficacy and elderly balance measurement tables were used to understand their fall status and its influencing factors;130 patients were randomly divided into a control group and an observation group,with 65 patients in each group.The control group was given routine community health services,and the observation group was given routine community health services+home visits.The two groups received a period of 12 months of intervention,the two groups were compared for balance,anti-fall knowledge,total behavior and efficacy scores,and number of falls per capita.RESULTS The logistic analysis showed that women,age,homeostasis,and chronic diseases were the risk factors that affected patients’falls,while fall prevention behavior and fall prevention effectiveness were their protective factors.Static balance,posture control,and homeostasis,the observation group was significantly better than the control group(P<0.01),and the observation group had significantly higher scores on anti-fall knowledge,behavior,and efficacy than the control group(P<0.01),and the average number of falls per capita was significantly lower than the control group(P<0.01).CONCLUSION According to the risk factors of falls,home visit intervention could effectively improve the patient’s ability to balance,increase the level of anti-fall knowing and trusting,and the effectiveness of preventing falls,and reduce the number of falls.It was worth popularizing and applying.
