摘要
目的探讨血清CRP联合SAA水平早期检测在婴幼儿脓毒血症中的应用价值。方法回顾性分析2018年7月-12月温州市人民医院儿童呼吸科确诊脓毒血症的患儿50例为观察组,对照组为同期因发热住院非脓毒血症患儿50例,健康组取同期在本院健康体检的健康儿童50例。追踪3组首诊时WBC、CRP与SAA水平。结果观察组、对照组、健康组WBC、CRP与SAA水平比较,差异有统计学意义(F值分别为70.34、124.00、1052.49,P<0.01);WBC计数和CRP水平比较,观察组和对照组均显著高于健康组(P<0.01),而观察组和对照组无统计学意义(P>0.05);SAA水平比较,观察组和对照组均显著高于健康组(P<0.01),而观察组显著高于对照组(P<0.01)。脓毒血症不同病程WBC、CRP与SAA比较,差异有统计学意义(F值分别为15.30、24.14、24.08,P<0.01)。脓毒血症不同病程WBC计数和CRP水平比较,≥3 d显著高于≤1 d和1 d^3 d(P<0.01),而≤1 d和1 d^3 d无统计学意义(P>0.05);血清SAA水平比较,≥3 d显著高于≤1 d和1 d^3 d(P<0.01),而1 d^3 d显著高于≤1 d(t=2.65,P<0.05);WBC、CRP与SAA均与脓毒血症呈正相关(r值分别为0.60、0.71、0.92,P<0.01);且与脓毒血症病程呈正相关(r值分别为0.59、0.61、0.71,P<0.01)。结论CRP和SAA水平均上升可早期提示脓毒血症。
Objective To observe the application value of early detection of C-reactive protein(CRP)and serum amyloid A(SAA)in serum of infants with sepsis.Methods Fifty children diagnosed as sepsis in Department of Children’s Respiratory from July 2018 to December 2018 in Wenzhou People's Hospital were analyzed retrospectively and taken as observation group.50 children hospitalized for fever and non-sepsis in the same period were taken as the control group and another 50 cases for physical examination at same period were taken as the healthy group.The levels of white blood cell,C-reactive protein and serum amyloid A at the first visit in the 3 groups were followed.Results The differences in the levels of white blood cell,C-reactive protein and serum amyloid A among observation group,control group and healthy group were statistically significant(F=70.34,F=124.00,F=1052.49,P<0.01);the count of white blood cell and the level of C-reactive protein were significantly higher in the observation group and the control group than that in the healthy group(P<0.01),but there was no statistical significance on the difference between the observation group and the control group(P>0.05).The levels of serum amyloid A in the observation group and the control group were significantly higher than that in the healthy group(P<0.01),while that in the observation group was significantly higher than that in the control group(P<0.01).There was statistical signif-icance on the differences of white blood cell,C-reactive protein and serum amyloid A in different course of sepsis(F=15.30,F=24.14,F=24.08,P<0.01).The count of white blood cell(WBC)and the level of C-reactive protein(CRP)in different course of sepsis were significantly higher in≥3 days than those in<1 day and 1 d-3 d(P<0.01),but there was no statisti-cal significance on the difference between<1 day and 1 d-3 d(P>0.05);and the level of serum amyloid A in≥3 days were significantly higher than those in<1 d and 1 d-3 d(P<0.01),while the level of serum amyloid A in 1 d-3 d were significantl
