摘要
目的 探讨穿透性角膜移植术后高眼压的影响因素及相应预防性干预措施.方法 选取58例穿透性角膜移植术患者,分析患者相关资料,总结穿透性角膜移植术后高眼压的影响因素及相应预防性干预措施.结果 58例穿透性角膜移植术患者中,发生术后高眼压15例(25.86%).不同年龄、性别、有无糖尿病史、有无再移植的穿透性角膜移植术患者术后高眼压发生情况比较,差异无统计学意义(P>0.05);有青光眼史、大泡性角膜病变、粘连性角膜白斑、单纯性角膜白斑、化学灼热伤、角膜变性及炎症期角膜溃疡患者穿透性角膜移植术后高眼压的发生率高于无上述病史或病变的患者(P<0.05).结论 穿透性角膜移植术患者发生高眼压与其原发疾病具有密切关系,对于青光眼、大泡性角膜病变、粘连性角膜白斑、单纯性角膜白斑、化学灼热伤、角膜变性、炎症期角膜溃疡的患者极易发生术后高眼压问题,护理人员应及时采用相关措施,从而降低穿透性角膜移植术后高眼压发生比例.
Objective To study the influence factors of high intraocular pressure after penetrating keratoplasty and the corre-sponding preventive interventions.Methods Fifty-eight patients with penetrating keratoplasty were selected to analyze relevant data and summarize the influence factors of high intraocular pressure after penetrating keratoplasty and corresponding preventive interven-tions.Results Of the fifty-eight patients,fifteen patients(25.86%)had high intraocular pressure.There was no statistically signifi-cant difference in the incidence of high intraocular pressure among patients after penetrating keratoplasty between patients of different a-ges,genders,history of diabetes,retransplantation(P>0.05).Patients with history of glaucoma,bullous keratopathy,adhesive corneal leukoplakia,simple cormneal leukoplakia,chemical burns,corneal degeneration,and inflammatory corneal ulcers have a higher incidence of high intraocular pressure after penetrating keratoplasty than those without previous medical history or diseased patients(P<0.05).Conclusion The occurrence of high intraocular pressure in patients with penetrating keratoplasty is closely related to the primary symptoms.For patients with glaucoma,bullous keratopathy,adherent leukoplakia,simple leukoplakia,chemical burns,cormeal degenera-tion,and inflammatory cormeal ulcer,it is very easy to have high intraocular pressure after surgery.Nursing staff should adopt relevant measures in time to reduce the proportion of high intraocular pressure after penetrating keratoplasty.
